İntrauterin İnseminasyon Sikluslarında, Rekombinant FSH ve Üriner FSH ile Yapılan Ovulasyon İndüksiyonunun Gebelik Sonuçları Üzerine Etkisi

The Effects of Ovulation Induction with Recombinant FSH and Urinary FSH on the Pregnancy Outcomes in Intrauterine Insemination Cycles

Özet

Amaç: Ovulasyon indüksiyonunda kullanılan rekombinant folikül stimulan hormon (rFSH) ve üriner folikül stimulan hormon (uFSH) ile yapılan ovulasyon indüksiyonu siklusları sonucunda oluşan gebelik oranlarını değerlendirmek ve istatistiksel olarak bir fark olup olmadığını saptamak.Gereç ve Yöntemler: Prospektif randomize kontrollü klinik çalışmaya, Bakırköy Kadın Doğum ve Çocuk Hastalıkları Eğitim ve Araştırma Hastanesi Etik Kurulu'nun 15.02.2010 gün ve 303 sayılı kararı ile başlandı. Klomifen sitrat tedavisi ile gebe kalamayan 105 infertil kadın çalışmaya dâhil edildi. rFSH grubunda 55 hastaya 90 siklus ovulasyon indüksiyonu ve intrauterin inseminasyon (İUİ), uFSH grubunda 50 hastaya 90 siklus ovulasyon indüksiyonu ve İUİ uygulandı. Stimulasyon süresi, kullanılan gonadotropin dozu, dominant folikül sayısı, hCG gününde endometrial kalınlık, siklus iptal oranları, OHSS insidansı, İUİ günü TMSS (toplam motil sperm sayısı), gebelik oranı, ilk trimester abort oranı, çoğul gebelik oranı değerlendirildi.Bulgular: Siklus başına gebelik oranları rFSH grubunda %16,7 ve uFSH grubunda %14,4 olarak saptandı. Her iki grup ararsında gebelik oranlarında anlamlı bir farklılık saptanmadı. Stimulasyon süresi, dominant folikül sayısı, hCG gününde endometrial kalınlık, siklus iptal oranları, OHSS insidansı ve ilk trimester abort oranları arasında fark saptanmadı.Sonuç: Çalışmamızın sonucuna göre, İUİ sikluslarında ovulasyon indüksiyonu için rFSH ve uFSH kullanımı ile benzer gebelik oranları elde edilmiştir. Ayrıca diğer parametreler arasında da fark bulunmamıştır.

Abstract

Objective: The purpose of this study is to evaluate outcomes of pregnancy after ovulation induction with recombinant Follicle Stimulating Hormone and urinary Follicle Stimulating Hormone and to determine whether there is a statistically significant difference between these treatment regimens. Material and Methods: This was a randomized controlled prospective clinical study. The Ethics committee approved our application with the decree 303 dated February 15th 2010. A total of 105 infertile women who failed to conceive with clomiphene citrate therapy were included in the study. A total of 90 cycles of ovulation induction with rFSH and IUI were performed on 55 patients of the rFSH group. A total of 90 cycles of ovulation induction with uFSH and IUI were performed on 50 patients of the uFSH group. Days of stimulation, dose of gonadotropins, number of dominant follicles, endometrial thickness on the day of HCG therapy, cycle cancellation rate, OHSS incidence, total motile sperm counts (TMSS) on IUI day, first trimester abortion ratio and pregnancy ratios per cycles, and multiple pregnancy rates were evaluated.Results: Pregnancy rates per cycles were 16.7 % in Gonal F, and 14.4 % in Fostimon groups, respectively. There was no significant difference between pregnancy rates. There is also no significant difference between days of stimulation, number of dominant follicles, endometrial thickness on the day of HCG therapy, cycle cancellation rate, OHSS incidence and the first trimester abortion ratio.Conclusion: According to the results of our study, pregnancy rates achieved were similar with rFSH or uFSH usage for ovulation induction in IUI cycles. Additionally, there is no significant difference in other parameters.
