Pediatrik Gartland Tip 3 Suprakondiler Humerus Kırıkları Cerrahi Tedavisinde Erken Dönem Sonuçlarımız

Özet
Amaç: Pediatrik üst ekstremite kırıklarında, potansiyel komplikasyonları nedeniyle önemli bir yer tutan suprakondiler humerus kırıklarında uyguladığımız cerrahi tedavinin fonksiyonel sonuçlarını değerlendirmektir. Gereç ve Yöntemler: Kasım 2010-Şubat 2012 tarihleri arasında kliniğimize başvuran, Gartland tip 3 suprakondiler humerus kırığı nedeniyle opere edilen hastalar retrospektif olarak tarandı ve çapraz kirshner teli ile osteosentez yapılmış olan 89 hasta çalışmaya dâhil edildi. Hastalar son kontrollerinde dirsek hareket açıklığı, deformite ve Flynn kriterlerine göre fonksiyonel ve kozmetik açıdan değerlendirildi. Bulgular: Hastaların son kontrollerinde yapılan eklem hareket açıklığı değerlendirmesinde, 86 hastada (% 96.7) iyi-mükemmel sonuç olduğu görüldü. Taşıma açısı ddeğerlendirmelerinde, 57 hastanın (% 64) taşıma açısı kaybı 5 derecenin altında (mükemmel), 26 hastanın (% 29.4) taşıma açısı kaybı 6-10 derece arası (iyi) olarak tespit edildi. Üç hastada takipleri sırasında redüksiyon kaybı olması nedeniyle revizyon yapıldı. İki hastada geçici radial sinir nöropraksisi görüldü. Bunun dışında hiçbir hastada ulnar sinir nöropraksisi görülmedi. Bir hastada cubitus valgus defomitesi gelişti ve sonraki dönemde deformite düzeltilmesi planlanarak takibe alındı. Sonuç: Literatürle uyumlu olarak çapraz kirshner teli ile tedavi ettiğimiz hastalarımızın fonksiyonel ve kozmetik sonuçlarının iyi olduğu sonucuna vardık.
Abstract
Objective: Because of potential complications, suprachondylar humerus fractures are major upper extremity injuries in the pediatric age group. The purpose of the study was to evaluate the functional outcomes of our patients who have been operated because of suprachondylar humerus fractures. Material and Methods: Between November 2010 and December 2012, patients who had been admitted to our clinic because of Gartland Type 3 suprachondylar humerus fractures were retrospectively reviewed and 89 patients who were operated with crossing Kirshner wires were included in the study. At the last control visit, the patients were evaluated as for range of motion, deformity, functionality according to the Flynn criteria. Results: At the last control, elbow ROM-s of the patients were evaluateed and in 86 patients (96.7 %) we had obtained good-excellent results. In the evaluation of carrying angle, 57 (64 %) patients had lower than 5 degrees loss (excellent), and 26 (29.4 %) patients had lower than 10 degrees loss (good). During follow-up, due to loss of reduction, revision surgery had been performed in 3 patients. We didn't experience any iatrogenic ulnar nerve neuropraxia. In one patient we had cubitus valgus deformity and planned deformity correction with further follow-up of this patient.Conclusion: In accordance with the literature we had good functional and cosmetic outcomes in the treatment of suprachondylar fractures with crossing Kirshner wires.
