SWL Results and Complications at Tokat Devlet Hastanesi

Özet

Amaç: Hastanemiz extracorporeal shock wave lithotripsy ünitesinde üreter veya böbrek lokalizasyonundaki taşlara yapılan extracorporeal shock wave lithotripsy sonuçlarımızı değerlendirmeyi amaçladık. Gereç ve Yöntem: Ekim 2011- Ekim 2012 tarihleri arasında renal veya üreteral taş tanısı ile extracor- poreal shock wave lithotripsy yapılan 81 hasta retrospektif olarak değerlendirildi. Ortalama takip süresi 33.5 gün idi. Extracorporeal shock wave lithotripsy uygulanan hastaların 50sinde böbrekte, 31inde ise taş üreterde lokalize idi. Extracorporeal shock wave lithotripsy ile tedavi edilen taşların pelvis, üst kaliks, orta kaliks yada alt kaliks olmasına göre lokalizasyon oranları sırasıyla %40, %26, %24, %10 iken üst üreter yada orta üreter lokalizasyon oranları ise sırasıyla %77.5, %22.5 olarak tespit edildi. Taşların odaklanması elipsoid odaklama özelliği olan c kollu skopi ile yapıldı. Tüm extracorporeal shock wave lithotripsy uygulamaları, Stonelith-V3 litotriptör ile aynı üroloji doktoru hekimi tarafından yapıldı. Her bir hasta için ortalama şok sayısı 2865, voltaj ise 19 kv idi. Bulgular: Ortalama 33.5 günlük takip süresi sonunda üreter ve böbrek de taşsızlık oranları sırasıyla %87 ve %82 olarak tespit edildi. Tartışma: Üriner sistem taş hastalığı teknoloji ilerlemesine rağmen extracorporeal shock wave lithotripsy ucuz ve etkili olması ile tedavide ön planda olmaya devam etmektedir.

Abstract

Aim: We aimed to evaluate SWL results performed on patients with renal andureteral stones in extracorporeal shock wave lithotripsy Unit of our hospital.Material and Method: 81 patients who have undergone extracorporeal shockwave lithotripsy due to renal or ureteral calculi between October 2011 -Oc- tober 2012 were evaluated retrospectively. Mean follow-up was 33.5 days.SWL was applied for 50 kidney stones and 31 ureteral stones. Localization ofstones in the urinary tract treated by extracorporeal shock wave lithotripsywas as follows: % 40, % 26, % 24 and % 10 were pelvic, upper caliceal,middle caliceal and lower caliceal calculi, respectively; whereas % 77.5 and% 22.5 were upper and middle ureteral calculi, respectively. The stones werefocused by a C-armed floroscopy having ellipsoid focus. All extracorporealshock wave lithotripsy applications were performed by same urology doctorafter a training period of one month using a Stonelith-V3 Lithotripter. Aver- age shock number was 2865 and average voltage was 19 kv for each case.Results: In 33.5 days follow-up, 82% and 87% of the patients with kidneyureteral calculi were stone-free, respectively. Discussion: In the treatment ofurinary stone disease; although there are new technological improvements,extracorporeal shock wave lithotripsy continues to be in the foreground dueto its cheapness and effectiveness.
