Preoperatif uygulanan albendazol tedavisinin karaciğer hidatik kist sıvısındaki etkinliği

Özet
Karaciğer hadatik kistinin tedavisi amacıyla bir çok ilaç kullanılmakla birlikte bunların etkinliği ve etki me¬kanizmaları henüz tam olarak aydınlatılmamıştır. Bu prospektif. randomize ve kontrollü çalışmada karaciğer hidatik kisti olan 30 olguda parazitin viabilitesi araştırı¬larak albendazolün etkinliği değerlendirildi. Bu amaçla onbeş olguda (Grup n,) preoperatif 21 gün süre ile 15 mg/kg/gün oral albendazol uygulanırken diğer 15 olgu (Grup n2) kontrol grubunu oluşturmaktaydı. Çalışmaya komplike olmamış ve ultrasonografik olarak evre 1,2,3 ve 4 kistler alındı. Tüm olgulardan peroperatif alınan kist sıvıları patolojik, mikrobiolojik, biokimyasal olarak analize edildi. Mikroskopik olarak çeşitli boyama yön¬temleri ile belirlenen canlı/ölü skoleks oranları alansal indeks yardımı ile yorumlandı ve albendazol kullanan grupta skoleks viabilitesinin anlamlı olarak azaldığı sap¬tandı (p = 0.001). Yapılan biokimyasal tetkiklerde de kist sıvısındaki glikoz, X GT ve ALP değerlerinde grup¬lar arasında anlamlı farklılıklar bulundu (p = 0.001, p = 0.005, p = 0.04). Sonuç olarak albendazolün preoperatif veya inope-rabl olgularda kullanımının, elde ettiğimiz veriler ışı¬ğında yararlı olacağı kanısındayız.
Abstract
The efficacy of preoperative albendazole on the activity of liver hydatid fluid Although several drugs have been used for the treatment of hydatid disease, none of them have been proved to be completely satisfying and their mechanism of action is still unclear. In this prospective, randomize and controlled trial scolocidal activity was assessed in 30 patients inorder to evaluate the effect of albendazole used preoperatively. Fifteen patients (Group ns) recived albendazole J5 mg/kg/day 21 days prior to operation whereas another 15 patients (Group n2) were selected as controls. Both groups were consisted of uncomplicated stage 1,2,3 and 4 (ultrasonographically) liver hydatid cysts. Cystic fluids were obtained from all patients pe im¬peratively for pathologic, microbiologic and biochemical analysis. Vital vs. nonvital scoleces were assessed using several dye techniques and results were evaluated by area index. Scolocidal viability was found to be signifi¬cantly lower in group it/ (p = 0.001) Significant differen¬ces were also observed between groups regarding glu¬cose, X GT and ALP levels of cystic fluid, (p = 0.001, p = 0.005, p = 0.04 respectively) In the view of our resuls, we concluded that the ad¬junctive use of albendazole preoperatively and in Inope¬rable hydatid disease seems beneficial.
