Özet

Kalite iyileştirmeye çaba harcayan hastanelerin, hastalar, o toplumda yaşayanlar ve hizmeti satın alanlar gibi kritik dış müşterilerinin ve doktorlar ve hemşireler gibi anahtar iç müşterilerinin bakış açısıyla kaliteyi değerlendirmeleri gerekmektedir. Bu araştırmanın amacı, bir devlet hastanesinin kalitesini, en önemli iç müşterilerinden olan doktor ve hemşirelerin bakış açısıyla değerlendirmektir. Araştırma 26 Haziran- 8 Ağustos 2003 tarihleri arasında Sakarya Devlet Hastanesinde gerçekleştirilmiştir. Bu hastanede çalışan 48 doktordan "Doktor Değerlendirme Sistemi Anketinin Kısa Formu" 81 hemşireden ise "Çalışan Değerlendirme Sistemi Anketinin Kısa Formu" ile veri toplanmıştır. Doktorlara göre hastanenin kalitesinin en kötü olduğu boyut "tıbbi donanım", en iyi olduğu boyut "doktorlar" dır. "Ünitelerde ve vardiyalarda yeterli sayıda hemşire bulunma derecesi" ise hastanenin kalitesinin en kötü olduğu madde olarak değerlendirilmiştir. Hemşirelere göre kalitenin en kötü olduğu boyut "maaş", en iyi olduğu boyut "doğrudan denetleyicileri/ilk amirleri" dir. Araştırmadan elde edilen bulgulara göre, yöneticilerin hastanede kaliteyi, artırmak için öncelikle hemşire sayısını artırmaya, yeterli tıbbi donanımı sağlamaya, taburcu sürecini, hemşirelerin; maaş, işe alma, yükselme, iş fırsatları ve iş ortamını iyileştirmeye çaba harcamaları gerekmektedir.

Abstract

Hospitals that try to improve quality should assess quality from the perspective of critical external customers, such as patients, community residents, and purchasers of care, and from the vantage point of key internal customers, such as physicians andnurses. The objective of this study is to assess quality of a general state hospital from the perspective of physicians and nurses. The study was conducted at Sakarya State Hospital between the dates of June 26 and August 8, 2003. Data were collected from 48 physicians who practice in this hospital using Short-Form Physician Judgment System Questionnaire, and from 81 nurses who work in the same hospital using Short-Form Employee System Questionnaire. From the perspective of physicians, the lowest ranked dimension of quality was "medical equipment", and the highest ranked dimension of quality was "physicians". "Extent to which units and shifts are adequately staffed" was ranked as the lowest item. From the nurse's perspective "salary" was the lowest ranked quality dimension and "their immediate supervisor" was the highest ranked dimension. According to the findings of this study, managers should give precedence to the efforts at increasing the number of nurses, providing adequate medical equipment, and improving discharge process, and salary, hiring, promotion, job opportunities, and work space of nurses in order to increase quality of the hospital.
