Özet

Profilaktik antibiyotik uygulaması, cerrahi kliniklerinde yaygın olarak kullanılmaktadır. Ancakantibiyotiklerin aşırı ve uygunsuz kullanımları, yan etkilerin ve dirençli bakterilerin ortaya çıkmasına;tedavi maliyetlerinin, morbidite ve mortalite oranlarının artmasına neden olmaktadır. Dolayısıylayerinde ve doğru bir antibiyotik kullanımının sağlanmasının sağlık kurumlarının etkin hizmetüretimindeki önemi büyüktür. Bu çalışmada, orta ölçekli bir kamu hastanesinde ameliyat öncesiyapılan profilaktik antibiyotik kullanım süreci incelenmiştir. Çalışma ile cerrahi antibiyotikprofilaksisi rehberi ile uygulamadaki uyumsuzlukların saptanması, beklenen ve gerçekleşenmaliyetlerin (ekstra maliyetlerin) hesaplanması amaçlanmıştır. Retrospekti f ve kesitsel olarak yapılançalışmada, 01.12.2012 ile 01.03.2013 tarihleri arasındaki 3 aylık dönemde Adanada bir kamuhastanesinde ameliyat edilen 760 hastanın preoperati f antibiyotik profilaksisine ait verileri kullanıldı.Profilaksi uygulamaları; antibiyotik seçimi, dozu, uygulama süresi ve uygulama zamanı bakımındansını flandırıldı. Mevcut profilaksi uygulamaları, hastanenin Enfeksiyon Kontrol Komitesincehazırlanmış cerrahi antibiyotik profilaksisi talimatlarını içeren rehber ile karşılaştırılarakdeğerlendirilmiştir. Yapılan uygulamaların ancak %24,74ünün (188 tanesi) rehber ile uyumlu,%75,26sının (572 tanesi) ise rehber ile uyumsuz olduğu saptanmıştır. En yüksek orana sahipuygunsuzluk kriterlerinin, gereğinden uzun süreli ve geniş spektrumlu antibiyotik kullanımı olduğutespit edilmiştir. Toplamda 2.371 kutu ve 8.570,45 TL maliyetinde antibiyotik kullanıldığı, ancakuygun olmayan profilaksi ile 4.579,13 TL ekstra maliyet ve 1.133 kutu antibiyotiğin gereksiztüketildiği görülmüş, hasta başına ortalama maliyetin ise 5,25 TL olması gerekirken 11,3 TL(beklenenin iki katındanfazla) olduğu saptanmıştır.

Abstract

Antibiotic prophylaxis is performed in surgical clinics frequently. Nevertheless, excessive andinappropriate usage of antibiotics leads to resistant bacteria and other side effects, as well as highercost of treatment, morbidity and mortality ratios. Therefore, establishment, surveillance andgovernance of appropriate antibiotics usage is very important in effective service of healthinstitutions. This study investigates the process of pre-surgical prophylactic antibiotics usage at the medium-scale public hospital. The study aims to identi fy the incompatibilities between the practicesand the surgical antibiotic prophylaxis guidelines, as well as calculate the expected and realized cost(extra cost). In this retrospective and cross-sectional study, we used the preoperative antibioticprophylaxis data of 760 patients who had surgeries between December 1st, 2012 and March 1st, 2013a Public Hospital in Adana. We investigated prophylaxis cases in terms of selection of antibiotics,dose, duration and time of the process. We compiled the current prophylaxis application methodsbased on the antibiotic prophylaxis guidebook, which was prepared by the Infection Control Committee according to the intra-hospital microorganism resistance pattern. We discovered that only24.74%, or 188, of the cases were in accordance with the guidebook, whereas 75.26%, or 572, of thecases were not in accordance. We found that the most frequent deviation from the guidebook isantibiotics usage for duration longer than necessary and for a spectrum broader than necessary. Wediscovered that 2,371 boxes of antibiotics, which cost TRY 8,750.45, were used in this period. Out ofthis, TRY 4,579.14 worth of 1,133 boxes were used unnecessarily. The resulting cost per patient wasTRY 11.30 and more than twice the fair value of TRY 5.25.
