Özet

Skolyoz tedavisindeki önemli sorunlardan birisi de ciddi eğriliği olan küçük çocukların tedavisidir. Bu çeşit eğrilkler eğer konservat'ıfyöntemlerle kontrol edilemiyorsa, seçilecek alternatif tedavi yöntemlerinden birisi subkutan rod uygulanmasıdır. Subkutan rod enstrümentasyonu ile tedavi edilen 16 hasta (7 kız, 9 erkek) retrospektif olarak değerlendirildi. Cerrahi işlem anındaki ortalama yaş 7 idi. On hastada idiopatik, 2 hastada nöromusküler skolyoz mevcut iken, 4 hastada skolyoz ile birlikte seyreden kompleks sendromlar mevcuttu. Hastaların Cobb yönetimi ile ölçülen preoperatif ortalama eğrilikleri 55 derece idi. Hastalar Moe1'nun tarif ettiği şekilde, füzyon yapılmadan subkutanöz olarak enstrümante edildiler. Cerrahi tedavi seçimi, 7 hastada konservat'ıf yöntemler denedikten sonra yapılırken, 9 hastaya ciddi eğrilikleri nedeniyle direkt cerrahi tedavi uygulandı, Kullaptjan enstrümanların 11 tanesi Harrington sistemi, 7 tanesi Isola sistemi ve 1 tanesi TSRHsistetni idi Ortalama 55 aylık bir takipten sonra 5 hastaya definitif tedavi olarak posterior füzyon uygulandı. Ortalama hasta başına düşen operasyon sayısı 4 idi. Son kontrolde (ortalama 70 ay} Cobb açısı ortalama 50 derece olarak belirlendi. Sekiz hastada toplam 14 komplikasyon gelişti (3 rod kırılması, 2 rod dislokasyonu, 3 enfeksiyon, 4 hook dislakgsyonu, 2 roda bağlı bası ülseri). Sonuç olarak, ciddi eğriliği ölüp halen büyüme potansiyeli önemli ölçüde devam eden ve eğriliğin progresyonu beklenen küçük çocukların edavi$inde, füzyon yapmadan uygulanacak subkutan rod tedavisi etkili bir alternatiftir.

Abstract

One of the major problems m treatment of scoliosis is the difficult curves of children that require immediate attention. Subcutaneous rod instrumentatiorjğ&CR) has been suggested for such curves that could nbrve controlled with conservative methods. Sixteen patients (7 girts and 9 boys) treated with SCR instrumentation were evaluated retrospectively. The average age at time of surgery was 7 years. Ten patients had idiopathic scalhsis', two had neuramuscular and the rest were patients that had complex syndromes associated with scoliosis. Thecpryes had an average Cobb angle of 55 degrees. The patients were instrumented with sûbcutaneoüsly placed rods without fusion as described by Moe. The surgical procedure was performed in 7 patients after a trial of conservative treatment and in 9 patients after the first-evaluation because of their significant deformity. The instruments that were used included 11 Harrington rods, 7koh rods and 1 TSRH rod. Four patients had a posterior fusion as a definitive surgery after an average of 55 months follow-up. The total number of operations per patient was 4. Af last follow up (average 70 months) the average Cobb angle was 50 degrees. Fourteen complications occurred in 8 patients (3 rod breakaqes, 2 rod dislocations, 3 infections, 4 hook dislocations, 2 pressure sores due to the rods). Conclusion: In small children with scoliosis that have a significant growth potential and risk for progression, SCR instrumentation without fusion is an effective alternative in management.
