Vaskülarize eklem grefti ile metakarp ve metakarpofalangeal eklem rekonstrüksiyonu

Özet

Eklem yaralanmalarımın tedavisinde cerrahın amacı; stabil, ağrısız ve yeterli hareket arkına sahip bir eklem sağlamaktadır. Eklem rekonstrüksiyonu için çeşitli yöntemler tanımlanmış olmakla birlikte, çocuklarda ve gençlerde bu yöntemlerin kullanım alanı sınırlıdır. Vaskülarize eklem transplantları ise büyüme potansiyeline sahip oldukları, için büyümenin devam ettiği hasta grubunda tercih edilmektedir. Serbest vaskülarize otojen metatars transferi ile aynı seansta yumuşak doku, ekstensör mekanizma, eklem yejleksör tendon rekonstrüksiyonu mümkün olmaktadır. Bir olguda, ı. metakarp, interfalengeal eklem, ve I. metakarp dorsumundaki yumuşak doku örtüsünün rekonstrüksiyonu, dorsalis pedis pediküllü vaskülarize II. metatarsofalengeal eklem kullanılarak yapılmıştır.

Abstract

To obtain a stable and painless, joint with a useful range of motion is the goal of the surgeon for the repair of destroyed joints. Although several methods have been described for joint reconstruction, applications of these techniques in children and young aldults are limited. Since vascularized joint transplants have the potential of growth, they are preferred in these group of patients in whom growth is taking place. Free vascularized autogenous toe transfer makes it possible the simultaneous reconstruction of$oft tissue, extensor mechanism, joint, and flexor tendon in complex hand injuries. In a case, missing first metacarpcal, interphalangeal joint, and soft tissue coverage of the dorsum of the first metacarpcal were reconstructed using vascularized second metatarsopalangeal joint based on dorsalis pedis artery.
