Determination of salivary risk factors and dental status in caries susceptible dental students: Two year longitudinal study

Özet
Bu klinik çalışmanın amacı, çürük aktivitesinin yüksek olduğu belirlenen dişhekimliği 2.sınıf öğrencilerinin tükürük teşhis testleri ile risk et¬kenlerini belirlemek, gerekli restorasyonların yapılmasından 2 ay ve 2 yıl sonraki ağız sağlı¬ğı durumlannda ve varsa risk etkenlerindeki değişimi izlemektir. Araştırma, yapılan klinik muayane sonucu aktif çürükleri olan (en az 4) ve tedaviye gereksinim duyan, DMFS indeksi yüksek (DMFS=21.83, s.s=11.19) ve yaşlan 18-20 arasında değişen 30 kişide yürütüldü. Klinik muayenede DMFT, DMFS ve CPITN in¬deksleri kullanıldı. İstirahat ve uyanlmış tükü¬rük akış oranları ölçüldü. Uyarılmış tükürükten MS (mutans streptokokları) ve LB (laktobasil-luslar) sayılarını saptamak amacıyla strip mu¬tans testi (Dentocult SM) ve dipslide testi (den-tocult LB), tükürüğün tamponlama kapasitesi¬ni ölçmek için de bir kolorimetrik strip testi olan Dentobuff kullanıldı.Her bireyin başlangıç çürük riski belirlendikten sonra yoğun bir teda¬vi planı uygulanarak, iki haftalık süre içinde, tüm çürük lezyonlan temizlenip, uygun resto¬rasyonlar yapıldı. Restoratif işlemlerin tamam¬lanmasından iki ay ve iki yıl sonra bireylerin klinik ve radyolojik muayaneleri ile tükürük testleri tekrarlandı. İki yıl sonunda bireylerin yarısında çürük oluştu. Bu süre içinde sadece 11 bireyde yeni çürük lezyonu oluşmadı.sonuç olarak uygun koruyucu ve restoratif tedavi planlamasıyla yeni çürük oluşumu minimalize edilebilir.bununla birlikte yüksek çürük riski taşıyan bireylerde tek başına restoratif tedavi çürük önlemede yeterli değildir.
Abstract
The purposes of the present longitudinal study were to (1) determine the caries risk factors in 30 second year dental students with high risk of dental caries by using diagnostic tests, and (2) find out the difference if exists, in oral he¬alth status and caries risk factors of the stu¬dents, two months and two years after the end of restorative treatment. 30 second year den¬tal students were selected from 112 students by clinical examination. The DMFS, DFS, DS scores and CPITN indices were recorded for each student. Stimulated and unstimulated flow rates for whole saliva were measured. Sa¬livary counts of SM and LB were determined from stimulated saliva using commercial tech¬niques: a strip mutans test Dentocult SM and a dipslide test Dentocult LB. Buffering capa¬city was estimated using a commercial colori-metric strip test Dentobuff. Each baseline sali¬vary finding of a subject was classified into ca¬tegories, either a high or low level for the ca¬ries related factor in question. All carious lesi¬ons were surgically removed and restorations were made in two weeks after determination of every individual's risk of caries. Clinical and radiographic examinations and salivary tests were repeated two months and two years af¬ter the end of restorative treatment. For statis¬tical analysis the paired sample t-test, Wilco-xon signed ranks test and chi square tests we¬re used. In the present study at the end of the two year period, half of the students had cari¬ous surfaces. Only eleven students did not de¬velop new caries lesions during that time. In conclusion, with a combination of appropriatepreventive and restorative treatment planning the indicence of caries could be minimized,whereas use of restorative treatment alone like the present study was not sufficient to prevent caries in these high caries risk subjects.
