Effect of different biomaterials on healing of large periapical osseous defects: A comparative evaluation by using computed tomography

Özet
Bu çalışmanın amacı büyük periapikal lezyonların enükleasyonlarından sonra yönlendirilmiş doku rejenerasyonu amacıyla poliglikolik asit ve laktik asit membranların ve solventlerle dehidrate edilmiş kemik allogreftlerinin (SDBA) kemik iyileşmesine olan katkısını düşük doz bilgisayarlı tomografi tekniği (BT) kullanarak saptamaktır. Defektler dört gruba bölünmüş olup A grubuna membranlar uygulanmış (n=10), B grubu defektlere ise SDBA kullanılmıştır (n=10). C grubu defektlerde (n=10) greft ve membranlar birlikte uygulanmış, D grubu defektlerde ise kontrol grubu olarak herhangi bir materyal kullanılmamıştır (n=10). Kemik kaviteleri operasyondan hemen sonra, postoperatif 3. ve 6. aylarda defektlerin derinliğinin, genişliğinin, yüksekliğinin ve BT numaralarının (greft densitesi) düşük doz BT tekniği kullanılarak ölçülmesiyle değerlendirilmiştir. A ve D grupları arasında kemik iyileşmesi açısından istatistiksel bir anlam saptanamamıştır. (p(0.05). Grup B ve C de iyileşme prosesi hızlı, tam ve kavitenin obturasyonu ile stabilizasyonu açısından üstün bulunmuştur. B ve C grupları arasında ise bir fark saptanamamıştır. Kontrol grubuyla karşılaştırıldığında SDBA uygulanan kavitelerdeki iyileşme olumludur. Ancak poliglikolik asit ve laktik asit membranların tek başına veya greftlerle kombine kullanımı açısından bir fark saptanamamıştır. Bir diğer sonuç ise BT teknikleri kullanılarak kemik iyileşmesinin tayininin ve greft canlılığının saptanmasının mükemmel düzeyde belirlenebileceğidir.
Abstract
The aim of this study was to investigate the effectiveness of polyglycolic and lactic acid membranes for guided tissue regeneration technique and solvent dehydrated bone allografts (SDBA) on bone healing after removal of large periapical lesions by using low dose computed tomography (CT). Defects were divided into four groups. In group A (n=10), membranes were applied, SDBA were used in group B (n=10). Membranes and grafts were applied into periapical osseous defects in group C (n=10) and finally, no material was used in group D (n=10) which served as control. Bone cavities were evaluated by measuring the widths, depths, heights and CT numbers (graft density) of the defects with low dose CT immediately following the operation, 3 and 6 months postoperatively. No significant differences were observed between group A and D (p>0.05). In group B and C healing process was rapid, complete, and stabilisation and obturation of the cavity was remarkably superior to group A and D. However, no difference in the healing ability was observed between group B and group C. SDBA on healing of periapical osseous defects were found successful compared with the control group. However, no additional healing was detected with the use of polyglycolic and lactic acid membrane alone or with bone graft. Also by using computed tomographic techniques, the serial evaluation of bone healing assessment and graft viability could be managed properly.
