An unusual gingival overgrowth associated with bone loss

Özet

Dişeti büyümeleri oldukça sık görülmesine rağmen periodontitisle birlikteliği nadir görülen bir durumdur. Maxilla ve mandibulanın sol tarafında aşırı dişeti büyümesi ile birlikte kemik kaybı ve gömülü dişler tesbit edilen ve başlangıçta küçük-büyük hücreli intermediate dereceli lenfoma rapor edilen 17 yaşında erkek olgu sunulmuştur.Tekrarlanan biyopsilerde hematolojik ve radyolojik tetkiklerde lenfoma olmadığı görülmüştür. Yapılan dental ve periodontal muayeneler sonucunda vaka, periodontitisle birlikte izlenen spesifik bir antitiyi temsil etmeyen dişeti büyümesi olarak tanımlanmıştır. Lezyonun histolojik görünümünde fibröz bağ doku zemininde plazma hücrelerinin hakim olduğu lenfoplazmositik hücre infiltrasyonu izlenmiştir. Üç yıllık takiple dişeti büyümesinde nüks ve kemik kaybında ilerleme görülmemiştir.

Abstract

Although gingival overgrowth is a common feature of gingival disease, the enlargement of gingiua with the association of periodontitis is a rare condition. The case of gingival overgrowth at the left side of maxilla and mandible and bone loss with the association of multiple impacted teeth in a 17 year-old male, which was initially diagnosed small and large celi intermediate grade lymphoma, was reported. Repeated biopsies, hematological and laboratory tests and radiological evaluations revealed the misdiagnosis of lymphoma. Dental and periodontal examinations demonstrated that the case was nonspecific gingival overgrowth with the association of periodontitis. The cause of the condition could not be identified. Histologically lesion contained fibrous connective tissue and lymphoplasmositic infiltration with the predominance of plasma cells. Three years of follow-up shouıed neither recurrence of gingival overgrowth nor progression of bone loss.
