Comparison of effectiveness of two different allogeic resorbable membranes used for guided bone regeneration in defective mandibles: An experimental study

Özet
Amaç: Bu çalışmanın amacını, patolojik lezyonların eksizyonu, tumor cerrahileri ve ortognatik cerrahiler gibi işlemler sonrasında oluşabilen cerrahi defekt bölgelerine çevre yumuşak dokudan fibröz bağdoku ve epitelyal materyalin göçünün engellenmesi amacı ile kullanılan iki farklı tipteki bioyolojik olarak rezorbe olabilen, allojenik esaslı membranöz greft materyallerinden solvent dehidrate Fascia Lata (SDFL) (BIODYNAMICS GmbH-Germany) ve solvent dehidrate Dura Mater’in (SDDM) (BIODYNAMICS GmbH-Germany) etkinliklerinin değerlendirilmesi ile yine bu membranların oluşan yeni kemiğin kalitesi üzerine etkilerinin değerlendirilmesi oluşturmaktadır.Gereç ve Yöntem: 42 tavşan; biri kontrol diğer ikisi ise deney olmak üzere toplam 3 gruba ayrılmıştır. Her deney grubu kendi içinde deney periyotlarına göre ikişer hayvandan oluşan 7 alt gruba ayrılarak her deney periodu için 6 mandibulada toplam 30 kavite oluşturulmuştur. Hayvanlar postoperatif 24 saat, 48 saat, 5 gün, 7 gün, 15 gün, 21 gün ve 45 gün sonunda sakrifiye edilerek rezeke edilen mandibulalar histopatolojik incelemeye alınmıştır. Histopatolojik değerlendirme dört parametrili bir skor sistemi kullanılarak yapılmış ve skor değerleri kaydedilmiştir. Sonuçlar istatistiksel olarak karşılaştırılmıştır. Bulgular: Çalışma sonuçları göstermektedir ki; kontrol grupları ile deney grupları arasında belirgin farklılıklar bulgulanmıştır. İyleşmenin az farkla da olsa SDFL grubu lehine olduğu tespit edilmiştir. Sonuç: Biyolojik olarak absorbe olabilen SDFL ve SDDM gibi kollejen esaslı allojenik membranlar, kontrol ile karşılaştırıldığında yeni kemik oluşumu üzerinde etkili oldukları tespit edilmiştir. SDFL ile iyleştirilen deneklerdeki sonuçların diğer gruplara oranla çok az da olsa yüksek oluşu bu memnran tipinin üstünlüğünü düşündürmektedir.
Abstract
Objective: The purpose of this study was to evaluate the effectiveness of two different types of bioresorbable, allogenic membranous graft materials; solvent dehydrated Fascia Lata (SDFL) (BIODYNAMICS GmbH-Germany) and solvent dehydrated Dura Mater (SDDM) (BIODYNAMICS GmbH-Germany) which were generally used to prevent the migration of epithelium and fibrous connective tissue into the surgically created defects such as excisions of pathological lesions, tumour surgeries, orthognathic surgeries or after traumatic injuries and also asses the effect of these membranous barriers on the quality of newly formed bone. Material and Methods: 42 rabbits were randomly assigned to three groups one for control and two for experiment. Each three group divided into 7 subgroups individually for different experimental periods. Thus each of the every experimental period contained 6 mandibles and 30 cavities. Animals were sacrificed postoperatively in 24 hours, 48 hours, 5 days, 7 days, 15 days, 21 days and 45 days and experimental sites were resected for histopathologically evaluation. During the histopathological study a score system which had four parameters was used to asses the healing features and also statistical analyse was utilized. Results: Either histopathological or statistical results demonstrated that the newly formed bone tissue was about the superior quality and quantity in the SDFL and SDDM groups when compared with the control groups. The highest degree of regeneration was obtained in defects underneath the SDFL membranous barriers.Conclusion: It is pointed out that biaoabsorbable collagen based allogenic membranes SDFL and SDDM when compared with the controls has a valuable effect on the amount of newly formed bone. SDFL groups seemed to be very slightly positive effects on healing of bone defects during the histopathological evaluation and this slight difference could not be identified.
