Özet

Sınıf II furkasyon defektlerinin rejeneratif tedavisinde trombositten zengin plazma (TZP) ve yönlendirilmiş doku rejenerasyonu (YDR) tekniklerinin birlikte ve ayrı ayrı etkin olduğunu gösteren araştırmalar vardır. Bu çalışmada sınıf II furkasyon defekti olan mandibular molar dişlerde trombositten zengin plazmanın YDR ile birlikte kullanımının klinik etkinliği incelenmiştir. Araştırmaya sınıf II furkasyon defekti olan sistemik olarak sağlıklı on gönüllü hasta dahil edilmiştir. Cep derinliği, ataçman düzeyi, dişeti çekilmesi, bukkal alveoler kemik yüksekliği ve horizontal sondlama derinliği klinik parametre olarak değerlendirilmiştir. Periodontal cerrahi sırasında altı olguda TZP+YDR, dört olguda YDR tek başına uygulanmıştır. Operasyondan altı ay sonra reentry işlemi sırasında aynı klinik parametreler değerlendirilmiştir. TZP+YDR grubunda cep derinliği, ataçman düzeyi, furkasyonun horizontal derinliklerinde operasyon öncesine oranla önemli değişiklikler izlenmiştir. Yalnız YDR uygulanan olgularda operasyon öncesi ve sonrasında önemli değişimler gözlenmemiştir. Her iki grup birbirleri ile karşılaştırıldığında aralarında önemli fark bulunmamıştır. Çalışmamızın sonuçlarına göre, sınıf II furkasyon defektlerinin rejeneratif tedavisinde, trombositten zengin plasmanın yönlendirilmiş doku rejenerasyonu ile kombine kullanımının cep derinliği azalması, ataçman kazancı ve furkasyon dolumu açısından olumlu sonuçlar verdiği ifade edilebilir.

Abstract

Autologous platelet-rich plasma (PRP) has been shown to be effective in the regenerative treatment of class II furcation defects. Guided tissue regeneration (GTR) is used in many periodontal regenerative procedures. This study was made to evaluate the effectiveness of two regenerative techniques, GTR and PRP+GTR for grade II molar furcation defects. Ten patients with class II furcation defects participated in this study. The primary outcomes of the study included changes in probing depth, attachment level, gingival recession, buccal alveolar bone height and horizontal depth as revealed by reentry surgeries at six months post-treatment. Six class II furcation defects were treated with PRP+GTR and four defects were treated with only GTR. Furcation defects treated with PRP+GTR exhibited significant gain in probing depth, clinical attachment level and horizontal depth of furcation but not in gingival recession and buccal alveolar bone height. There was no significantly additional gingival recession. Furcation defects treated with GTR alone exhibited no significant differences between baseline and six month postoperative values. Also, there were no significant differences between two groups when compared before and after surgery. The results of this study suggest that PRP+GTR treatment model may improve probing depth, clinical attachment and horizontal depth of class II furcation defects in humans.
