Özet
Giriş: Özellikle kas iskelet sisteminin uygunsuz kullanımı sonucu ortaya çıkan kronik ağrılar toplumun genel sağlık sorunları arasındadır. Bazı meslek grupları kas iskelet sistemini uygun olmayan şekillerde kullanmaya bağlı olarak kronik ağrı sorununu sık yaşamaktadır. Bu meslek gruplarından biri de dişhekimleridir. Amaç: Bu çalışmanın amacı, bir ağız-diş sağlığı merkezinde çalışan dişhekimlerinde kas iskelet sistemi kaynaklı ağrı sorununun boyutu ve SF-36 ile yaşam kalitesine etkisinin saptanmasıdır. Gereç ve Yöntem: Araştırma, tanımlayıcı tipte epide-miyolojik bir çalışma olup, 115 dişhekimine üç bölümden oluşan 27 soruluk anket formları dağıtılmış, anketi 81 dişhekimi yanıtlamıştır. Ağrı yakınmasının şiddeti Görsel Eşdeğerlik Ölçeği (VAS) ile, sağlıkla ilgili yaşam kalitesi ise SF-36 ile değerlendirilmiştir. Bulgular: Katılımcıların %59.3'ü 30-39 yaş grubunda, %82.7'si kadın, %79'u halen evli, %42'si halen sigara içmekte olup, %66.7'si ise haftada üç kez en az 30 dakika sürecek şekilde egzersiz yapmamaktadır. Hekimlerin %88.9'u Beden Kitle İndeksi yönünden normal sınırlardadır. Ortalama çalışma süresi 10.7 yıldır. Tam zamanlı çalışanların oranı %95.1'tir. Ortalama çalışma süresi hafta içi 8.3, hafta sonu 6.1 saat; hasta başında geçirilen ortalama süre hafta içi 5.1, hafta sonu 4.9 saattir. Katılımcıların %64.2'sinin halen ağrısı olup, en fazla boyun, bel ve omuz ağrısı görülmüştür. SF-36 puanı yönünden en yüksek puanın 84.074 ortalama ile fiziksel fonksiyonellik, en düşük puanın ise 51.543 ile canlılık bileşenine ait olduğu saptanmıştır. Halen ağrısı olanların %82.7'sinde ağrı şiddeti VAS skalasına göre 1-5 arasındadır. Yaş, meslekte çalışma süresi ile ağrı arasında anlamlı ilişki bulunmuştur. SF-36 ölçeği bileşenlerinin ortalamaları, ağrı sorununun bulunmasına ve ağrı şiddetine qöre değerlendirildiğinde, sekiz bileşenden altısı için aradaki fark istatistiksel yönden anlamlı bulunmuştur. SONUÇ: Çalışma bulgularına göre, dişhekimlerinin çalıştıkları merkezdeki poliklinik odalarındaki' çalışma koşullarının düzeltilmesi, muayene kabinlerindeki ergonomik koşullarla ilgili düzenlemelerin yapılması (gürültü, aydınlatma, hasta birikimi, aletler ve dinlenme yerleri vb), hasta muayeneleri arası sürenin arttırılması, dengeli beslenmenin, düzenli egzersizin yaşamda yer alması, hekimlere ağrıyla başa çıkmayla ilgili eğitim verilmesi, ağrı sorunu olan dişhekimlerine uygun tedavi yöntemleriyle yaklaşılması, ağrı sorunun önlenmesi, giderilmesi ve hafifletilmesinde önemli adımlardır
Abstract
Introduction: The dental team is at high risk of neck and back problems due to the limited work area and impaired vision associated with the oral cavity. These working restrictions frequently cause a clinician to assume stressful body positions to achieve good access and visibility inside the oral cavity. Furthermore, dental procedures are usually long and require much more concentration during work. Back pain is one of the most common and trouble¬some of complaints; and has been cited among the most common reasons for missing work. Objective: In this study, dimension of musculo-skelatal system pain and its effect on health related quality of life (SF-36) among dentists in a oral-dental health center was investigated. Material and Method: In this descriptive study, no sampling procedure was used. The questionnaire consisted three sections and 27 questions was administered to the study group of 115 dentists while only 81 of them replied. Pain severity is measured by Visual Analogue Scale and health related life quality is assessed by SF-36 sacle. Results: Of the study group, 59.3% were 30-39 age group, 82.7% were women, 79% were currently married, 42% were currently smoker, 66.7% were not exercising at least three times a week for halfan hour, 88.9% were between normal limits in terms of Body Mass Index. Average duration of working life was 10.7 years. Full time workers' ratio was 95.1%-Average duration of work hours was 8.3 hours for weekdays and 6.1 hours for weekends. Average duration of patient time was 5.1 hours for week¬days and 4.9 hours for weekends. 64.2% of study group complained pain currently that was located in neck, shoulder and waist. Highest SF-36 score was found as 84.074 average as physical functioning while the lowest one was 51.543 average as vitality component. Of the study group, 82.7% had VAS value (pain severity) between 1 and 5. A significant relationship was found between current pain and age and work life duration. When averages of SF-36 scale components were analysed according to existency of pain and severity of pain, a statistically significant relation was found for six components out of eight. Conclusion: The findings of this study showed that, in this dental center, outpatient room work conditi¬ons should be re-arranged in terms of physical conditions such as noise (both dental units' and patients'), illumination, dental equipment accessibility, resting time between consecutive patient appointments; ergonomics of the work space and physical facilities of the rest room. On the other hand, dentists should adopt necessary life-style changes such as balanced nutrition and regular exercise in order to decrease the burden of pain. This group of professionals also has to be trained on how to tackle with pain. It is expected that with these changes the productivity and life quality will be increased.
