Bilateral sagittal split ramus osteotomisi ile mandibuler setback sonrası kondil pozisyonunun değerlendirilmesi

Özet
Giriş: Mandibuler prognasiye sahip hastaların tedavisinde bilateral sagittal split osteotomisi (BSSO) sıklıkla uygulanan bir yöntemdir. Amaç: Amacımız; BSSO sonrası kondil pozisyonunda meydana gelen değişikliklerin belirlenmesidir. Bireyler ve Yöntem: Araştırmamızda Le Fort I cerrahisiyle birlikte veya tek başına BSSO uygulanmış 15 olguda cerrahiden hemen önce (T1), cerrahiden 1 ay sonra (T2), ve cerrahiden 6 ay sonra (T3) alınan lateral sefalometrik, anteroposterior, baziller ve eklem radyografları kullanılarak kondil pozisyonundaki değişiklikler incelenmiştir. İstatistiksel analiz Tek Yönlü Tekrarlayan Ölçümler Dizaynı, Friedman Analizi ve Bonferroni düzeltimli Wilcoxon analizi kullanılarak yapılmıştır. Bulgular: Mandibuler geri alma miktarı ortalama 3.86mm, T2’den T3’e anterior mandibuler hareket miktarı ortalama 1.56 mm olarak belirlendi. İki zaman aralığında sağ ve sol kondillerin mediolateral, superior-inferior ve rotasyonel hareketleri istatistiksel olarak anlamlı değildi (p >0,05). Sonuç: Araştırmamızda kondili içeren proksimal segmentin; dolayısıyla kondilin; uygulanan cerrahi protokol ile stabil kaldığı bulunmuştur
Abstract
Introduction: Bilateral sagittal split osteotomy (BSSO) is widely used in orthognathic treatment of mandibular prognathia patients. Aim: Our aim was to determine the changes in condyle position after BSSO. Subjects and Method: Our study consisted of 15 patients who had BSSO with or without Le Fort I surgery. Lateral cephalograms; anteroposterior, basillary and transcranial radiographs taken before surgery (T1), 1 month after surgery (T2) and 6 months after surgery (T3) were used to evaluate the changes in condyle position. Statistical analysis was carried out using One-way repeated measures design, Friedman analysis and Wilcoxon analysis with Bonferroni correction. Results: The average amount of mandibular setback was 3.86mm, and the average mandibular anterior movement from T2 to T3 was 1.56 mm. Mediolateral, superoinferior and rotational movements of the right and left condyles were not statistically significant (p >0,05). Conclusion: The results of this study showed that the surgical protocol used in this study maintains the position of the proximal segment and the condyle.
