Özet

Giriş: Diabet periodontal hastalık için bir risk faktörüolmakla birlikte periodontal hastalık diabetinbir komplikasyonu olarak kabul edilmektedir. İç içegeçmiş bu sebep sonuç ilişkisi çerçevesinde diabetinkonak cevabında neden olduğu değişimler ve budeğişimlerin periodontal hastalık oluşumundaki rollerihala bir araştırma alanı oluşturmaktadır.Amaç: Çalışmamızda, diabetik koşullarda oluşanperiodontal inflamasyon ve doku yıkımının yine periodontitistegörülen periodontal inflamasyon ve dokuyıkım düzeyleriyle karşılaştırılmalı olarak incelenmesiamaçlandı.Gereç ve Yöntem: Otuz üç adet Wistar türü sıçan 3gruba bölünerek 11 sıçanda streptozotocin enjeksiyonuylageri dönüşümsüz deneysel diabet (grup 1),11 sıçanda ligatürle aşırı plak birikimi sağlanarakdeneysel periodontitis (grup 2) oluşturuldu ve 11sıçan negatif kontrol grubu (grup 3) olarak kullanıldı.Periodontal inflamasyon ve doku yıkımı çalışmabölgesindeki dişeti IL-1düzeyi, dişeti inflamatuvarhücre infiltrasyonu ve marjinal alveoler kemik kaybımiktarı ile değerlendirildi. Deneklerden alınan dişetiörneklerinde ELISA yöntemi ile IL-1düzeyleri ölçüldüve histometrik olarak inflamatuvar hücre sayımıyapılarak inflamatuvar hücre infiltrasyonu belirlendi.Sakrifikasyonlardan sonra çıkarılan çenelerde iseçalışma bölgesindeki marjinal alveolar kemik kaybımiktarı lineer ölçümlerle kantitiye edildi.Bulgular: IL-1seviyesi grup 1 ve 2’de grup 3’tenyüksek bulundu ve bu farkın istatistiksel olarak anlamlıolduğu saptandı (p0.05). İnflamatuvar hücre infiltrasyonuve marjinal alveol kemik kaybının grup 1 ve2’de grup 3’den daha fazla olduğu görüldü. Grup 1ve 2 ile grup 3 arasında istatistiksel olarak anlamlı birfark varken (p0.05).Sonuç: Bulgularımız, diabetin tek başına periodonsiyumdakonak cevabını değiştirebileceği, periodontaldokularda yıkıma neden olabileceği ve periodontalsağlığı etkileyebileceği fikrini desteklemektedir. Ancakdeneysel diabetik koşulları dışında kontrol altındakidiabette de periodontal durumun bu anlamda kıyaslanacağıbaşka çalışmalara ihtiyaç vardır.

Abstract

Introduction: Diabetes is a risk factor for periodontal disease, whereas periodontal disease has been accepted to be one of the complications of diabetes. Within the limits of this integrated relationship, the alterations in host response due to diabetes and the roles of these alteretarions in the periodontal disease developement have still been a research area. Aim: In our study, it was aimed to comparatively investigate the periodontal inflammation and tissue destruction in diabetic conditions together with the periodontal inflammation and tissue destruction levels in periodontitis. Material and Method: Thirtythree Wistar rats were divided into 3 groups; irreversible experimental diabetes was created by streptozotocin injection in 11 rats (group 1), experimental periodontitis was created by ligature induction in 11 rats (group 2) and the other 11 rats were utilised as the negative control group (group 3). Periodontal inflammation and tissue destruction were assessed via gingival IL-1β level, gingival inflammatory cell infiltration and marginal alveolar bone loss in the study regions. The IL-1β levels were measured by ELISA and the inflammatory cell infiltration was determined by histometric inflammatory cell counts in the gingival samples taken from the subjects. Following sacrifications, the mandibleswere removed and the marginal alveolar bone loss was quantified by linear measurements in the study regions. Results: IL-1β levels were higher in group 1 and 2 than in group 3 and the differences were statistically significant (p0.05). Inflammatory cell infiltration and marginal alveolar bone loss were found to be higher in group 1 and 2 than in group 3. While the differences between group 1 and 3, and group 2 and 3 were statistically significant (p0.05). Conclusion: Our results suggest that diabetes alone may alter the host response in periodontium, may lead to periodontal tissue destruction and may influence the periodontal health. However, further studies are needed to compare the periodontal status also in metabolically controlled diabetes, other than the experimental diabetic conditions.
