Oral kavitede helikobakter pilori varlığının araştırılması: Olası gastrointestinal sistem hastalıkları ve ağız sağlığı ilişkisinin saptanması (pilot çalışma)

Özet
Amaç: Helikobakter pilori; gram-negatif, mikroaerofilik, S ve rod şekilli bir bakteridir. Dünya popülasyonun yaklaşık % 50’sini etkiler1. Gastrointestinal infeksiyonlara ve mide kanserlerine yol açabilir. Primer lokalizasyonu gastrik mukoza olan bakterinin özellikle tekrarlayan, ısrarcı infeksiyonlarda sekonder odağının ağız mukozası olabileceği düşünülmüştür. Fakat, bakterinin ağız mukozasında hangi şartlarda varolup, oral florada yaşadığı tam olarak bilinmemektedir. Çalısmanın amacı hızlı üreaz testi kullanarak, oral mukozada helikobakter pilori varlığını tespit etmek ve varlığını etkileyen şartları incelemektir. Bireyler ve Yöntem: Çalışmada bakterinin, bulunma olasılığı yüksek olan dental plaktan ve diştaşından uzak sağlıklı oral mukozadaki görülme sıklığı, rapid üreaz testi (CLOtest Rapid Urease Test, Kimberly-Clark, Roswell-USA) kullanılarak araştırıldı. Hastaların, cerrahi işlem öncesi oral hijyen, sağlık bulguları, gastrointestinal sağlık bilgileri ve geçmişi kaydedildi. Test minör cerrahi uygulanan 40 hastadan alınan oral mukoza örnekleri ile yapıldı ve üreaz testinin 24 saatlik değişimleri kaydedildi. Elde edilen sonuçlar istatistiksel olarak incelendi. Bulgular: Hastaların % 50’sinde (20 hasta) test pozitif sonuçlandı. Oral hijyen eksikliğinde bakteri görülme yüzdesinin istatistiksel olarak anlamlı arttığı gözlendi. Gingivitisi bulunan 16 hastanın 11’inde (% 68,8) sonuç pozitifken, bu sonuç periodontitisli hastalarda % 100’dü (5 hasta). Sigara kullanan 31 hastanın 22’sinde (% 71,4) sonuç pozitifken, sigara kullanmayanlarda bu oran % 45,5 olduğu tespit edildi. Sonuç: Sonuçlarımız oral hijyen ile helikobakter pilori varlığı arasında istatistiksel anlamlı bir ilişki bulunduğunu ortaya koyarken; sigara kullanımı ve önceden geçirilmiş gastrointestinal rahatsızlığı olan gruplarda bakteri varlığı görülme oranı artmış olmasına rağmen bu sonuçların istatistiksel olarak belirgin olmadığı tespit edildi. Sonuç olarak test sonuçları doğruluk oranı daha yüksek testler ve daha yüksek katılımlı araştırmalar yapılarak Helikobakter pilori’nin oral mukozadaki varlığı incelenmeli ve gastrointestinal rahatsızlıkların tedavisi üzerindeki etkileri araştırılmalıdır.
Abstract
Aim: Helicobacter pylori is a gram-negative, microaerofilic, S and rod shaped bacteria. It effects nearly 50 % of the world population1. It can cause gastrointestinal infections and gastric cancer. Secondary focal point of recurrent and persistent infections caused by this bacteria whose primary localization is the gastric mucosa, is thought to be the oral mucosa. However in which conditions the bacteria appears or survives in the mouth flora is not clear. The aim of the study is to ascertain the existence of Helicobacter pylori on oral mucosa using a fast urease test and assess the conditions contributing to its presence. Subjects and Method: In our study; existance of bacteria on healthy oral mucosa, far from dental plaque and calculus that is more likely to host bacteria, was evaluated with rapid urease test (CLOtest Rapid Urease Test, Kimberly-Clark, Roswell-USA). Data of the patients concerning their oral hygiene levels, medical and gastrointestinal findings were recorded prior to the surgical procedure. The test was applied to oral mucosa specimens, that were taken from 40 patients who underwent oral minor surgery and alterations of the urease tests in 24 hours were recorded. Result: The tests were positive on 50 % (20 patients) of the patients. A statistically increased level of Helicobacter pylori existence was observed in patients with poor oral hygiene. While 68,8 % of the results were positive among patients with gingivitis (11 of 16 patients), 100 % of the tests were confir-med positive among patients with peridontitis (5 patients). While the tests were positive in 22 of 31 patients who were smokers (71,4 %), this rate was 45,5 % in non-smoking patients. Conclusion: While our results indicate a statistically significant correlation between oral hygiene and the existence of Helicobacter pylori, no statistically significant relation was confirmed between the existence of Helicobacter pylori and smoking habit or previous history of gastrointestinal disorders even though the results were relatively higher among these patients. As a result, the presence of Helicobacter pylori on oral mucosa need to be evaluated with more dependable tests and with a higher amount of attendance and its effects on treatments of gastrointestinal disorders need to be researched.
