Özet

Amaç: İki implant destekli overdenture’larda bar ve topuz tutucuların ve üç farklı kret yüksekliğinin, implantlardaki gerinime etkisini araştırmaktır. Gereçler ve Yöntemler: Kanin bölgelerinde rozet gerinim ölçer yerleştirilmiş iki implantı bulunan yüksek rezidüel kretli dişsiz mandibular akrilik rezin model, mukozayı taklit eden 1.5 mm’lik plakayla kaplandı. Altışar adet bar ve topuz tutuculu overdenture hazırlandı. Protezler modele yerleştirilerek 100, 150, 200, 300, 400 N statik kuvvetle üçer kez posteriordan dikey yönde çift taraflı, eşzamanlı yüklendi. İmplantlardaki gerinim ve mukozayı taklit eden plakadaki deformasyon miktarı eşzamanlı olarak kaydedildi. Protez kenarları kısaltılarak orta ve düşük kret yüksekliklerini temsil eden protezler hazırlandı. Yükleme ve kayıt işlemleri tekrarlandı. Elde edilen gerinim verileri ve deformasyon miktarları tutucu tipleri arasında ve kret yükseklikleri arasında karşılaştırıldı. Abstract

Aim: The purpose of this study was to investigate the strains induced on implants, supporting mandibular overdentures, with respect to bar and ball attachment types and three different levels of alveolar bone resorption. Materials and Methods: Two rosette strain gauges were bonded on the collar of two implants and were incorporated into the canine regions of an acrylic resin experimental model that was coated with 1.5 mm thick elastomeric tissue mimic material. Six samples were constructed for each of the ball and bar attachment retained configurations. Specimens were loaded up to 400 N static axial force. For each loading sequence, strain induced on implants and deflection of mucosa simulant was recorded. Residual ridge height was altered by means of index moulds and loading - recording sequences were repeated for different residual ridge heights. Strain values and amount of deformation were assessed. Results: Overdentures with ball attachments caused statistically significantly higher strain on implants and lower amount of deflection on tissue-bearing area than overdentures with bar attachments (p0.05). Conclusion: Attachment type used effects the load distribution between implants and edentulous ridge. Tension was observed at the lingual sides of the neckof the implants. Therefore, it can be predicted that, when loaded vertically in in vitro conditions, an anterior component of force is formed on the dentures, related with the anatomy of alveolar ridge.
