Dorothea Elizabeth Orem’in Öz –Bakım Teorisi’ne Göre Konjestif Kalp Yetersizliği Olan Hastanın Hemşirelik Bakım Planı

Özet
Hemşirelik kuram ve modellerinin hemşireliğineğitim, yönetim, uygulama ve araştırma alanlarındakullanılması, bu alanlara kavramsal bir çerçeve sunaraktemel oluşturmakta ve rehberlik etmektedir. Orem’inÖz Bakım Eksikliği Hemşirelik Kuramı, yaşamınsürekliliği için gerekli olan öz bakıma neden ihtiyaçduyulduğunu açıklar. Öz bakım, bireylerin yaşamlarıboyunca, sağlık ve iyilik durumlarını sürdürebilmeleriiçin üzerlerine düşen görevleri yapmalarıdır. Orem’egöre hemşireliğin amacı; bireyin kendi bireyselbakımını yapabilecek duruma gelebilmesi içinyardımcı olmak ve kendi bireysel bakımınıüstlenmesini sağlamaktır. Konjestif kalp yetersizliği(KKY), organizmanın metabolik ihtiyaçlarınıkarşılayacak yeterli kardiyak debiyi sağlayamamasıdurumudur. Kalp yetersizliğinde hemşirelik bakımınınamaçları; komplikasyonları önlemek, tedavininetkilerini değerlendirmek, egzersiz toleransınıarttırmak, hasta/aile eğitimidir. Bu olgu sunumunda“Konjestif Kalp Yetersizliği” tanısı ile koroner yoğunbakım ünitesinde izlenen hastanın Orem’in “Öz BakımTeorisi’ne” göre öz bakım gereksinimlerinin belirlenip,kuramın her basamağına göre hemşirelik bakımplanının sunulması amaçlanmıştır.
Abstract
The usage of the nursing Theory and models in the areas of education, management, application of nursing provides a basis and guides to these areas by providing an institutional framework. Orem's Self-Care Deficit explains why nursing theory is needed for the continuity of life. Self-care is the task of individuals to fulfill their lives, health and well-being. According to Orem, the purpose of the nursing is assisting to the person until able to doing her/his self-care and providing ensure taht his / her individuals care is undertaken as soon as possible. Congestive heart failure (CHF) is the inability to provide sufficient cardiac output to meet the metabolic needs of the organism. The aims of nursing care in heart failure is preventing complications, evaluating the effects of treatment, increasing exercise tolerance, patiden/family education. This case report was planned to determine the self-care needaccording to Orem's "Self-Care Theory" and presenting it in the nursing care plan of the patient with "Congestive Heart Failure" Diagnosis following in coronary intensive unit. Each step of thetheory is summarized with the patient's data.
