Özet

Ekstrüziv lüksasyon, dental travmalar içerisinde yer alan ciddi yaralanmalardan biridir. Ekstrusiv lüksasyona uğramış dişte zamanla nekroz ve enflamatuar resorpsiyon görülebilir. Ancak bu durum, uygun tedavi yaklaşımlarıyla önlenebilir. Bu olgu bildirimi, sol üst keser dişinde ekstrusiv lüksasyonu ve sağ üst keser dişinde de komplike kron kök kırığı olan 14 yaşındaki erkek bir hastaya ait tedavi planını göstermektedir. Sol üst keser diş, lokal anestezi altında hafif parmak basıncıyla yerine yerleştirilip yarı esnek bir splint ile stabilize edildi. Sağ üst keser diş ise iki hafta sonra kron kök kırığına bağlı olarak çekildi. Sol üst keser dişe iki haftalık kalsiyum hidroksit uygulamasını takiben kök kanal tedavisi uygulandı. Sağ üst keser diş yerindeki boşluğun estetik olarak kapatılması amacıyla geçici bir protez yapıldı. Bir yıl sonraki değerlendirmesinde dişte herhangi bir problem tespit edilmedi.

Abstract

Extrusive luxation of permanent teeth is one of the most serious injuries in dental traumatology. These teeth may undergo necrosis and inflammatory resorption. However, these conditions can be prevented with suitable treatment approaches. This case report describes the treatment of a 14 year-old male patient who has a maxillary left central incisor with extrusive luxation and a maxillary right central incisor with complicated crown-root fracture. The patient was referred to our clinic 24 hours after the bicycle accident. The treatment procedure of left central incisor was consisting of repositioning the extruded tooth by gentle finger pressure under local anesthesia, making a semi rigid splint. Right central incisor was extracted after two weeks. Then root canal therapy was performed on the left incisor after the calcium hydroxide based root canal dressing was placed for two weeks. A temporary removable partial denture was then constructed to reestablish esthetics, caused by the loss of the maxillary right central incisor. After one year follow up, no sign or symptom of a problematic situation was detected in the clinic or radiographic evaluation.
