Özet
Replantasyonda başarıyı etkileyen faktörleri tipik ola¬rak yansıtan iki olgu değerlendirildi. Birinci olgumuz olan onbir yaşındaki erkek hastanın anamnezinden, 2.5 sene önce sol üst 1. nolu dişinin kaza sonucu avulse olduğu ve avulse olan dişin kuru bir ortamda dişhekimine götürüldüğü anlaşıldı. Dişhekimi tarafın¬dan replante edilen, 2.5 ay rijit fiksasyon uygulanan ve fiksasyonun çıkarılmasını takiben kanal dolgusu yapılan dişte 2.5 sene sonra replasman rezorbsiyonu ve kırık hattı gözlenmiştir. İkinci olgumuz olan ve sol üst orta kesici dişine be¬yazlatma uygulamak istediğimiz .42 yaşındaki erkek hastanın bu dişinin 25 sene önce kavga nedeniyle avulse olduğu ve dişi musluk suyunda yıkadıktan sonra kendisinin replante ettiği anamnezinden anla¬şıldı. Alınan radyografta, dişin pulpa boşluğunun ge¬niş ve apeksinin kapalı, çevre dokuların ise sağlıklı olduğu görüldü. Dişhekimi tarafından replante edilen bir olguda başarısızlık söz konusu iken, hastanın ken¬disinin uyguladığı replantasyonun başarılı olması tar¬tışıldı ve başarıyı dişin alveol dışında kalma süresi, bu süre içinde periodontal liflerin canlı tutulması, nonrigit kısa süreli fiksasyon gibi faktörlerin etkilediği vurgu¬landı.
Abstract
Two cases which reflect typically influence success in replantation were evaluated. Medical history taken from an eleven-years old male, it was understood that his left-upper central tooth had become avulsed accidentally two and a half years ago, and it had been taken to the dentist in a dry atmosphere. Theavulsed tooth was replanted and stabilized for two and a half months with a rigit splint and after removal of the splint, canal filling was carried out by the dentist. After two and a half years, a fractured line formed due to the replacement resorbtion was obser¬ved. Medical history taken from 42 years old male it was understood that the patient's leftupper central tooth had become avulsed 25 years ago during a fight. He had reimplanted on his own, after washing it with tap waiter. It was seen on the radiograph that the pulp cavity of the tooth was wide, the apex was closed and surrounding tissues were healthy. While the rep¬lantation carried out by the dentist was unsuccesful, the one done by the patient himself was successful. This situation was discussed and was argued that factors such as extraalveolar time of the tooth, the preservation of the vitality of the periodontal ligament cells, and minimal non-rigit splinting had led to the success.
