Özet

GnRH agonistleri ile oluşan hipoestrojenizmin kemik mineral içeriği (Bone mineral content-BMC) üzerine etkilerini araştırmak için, 20 endometriozisli olgu 1 yıl boyunca incelenmiş ve sonuçlar retrospektif olarak incelenen Danazol grubu ile karşılaştırılmıştır. 6 ay, 3.75 mg/28 gün triptorelin verilen grupta başlangıç¬ta 1.080 ± 0.040 g/cm2 olan BMC, 6 aylık tedavi so¬nunda 0.985 ± 0.033 g/cm2 'ye düştü. Tedaviden sonraki 6. ayda ise, ancak 1.015 ± 0.026 g/crn2 ye kadar yükseldi. Tedavi ile belirgin derecede artan üriner Ca/kreatinin oranı tedavi kesildikten sonra aza¬lırken, serum alkalen fosfatazı yüksek düzeyde sey¬retti. 6 ay, 800 mg/gün danazol verilen grupta estradiolün (E^ erken folliküler faz sınırlarına düşme¬sine rağmen BMC kaybı izlenmedi. Bu durum dana-zolun androjenik etkisine bağlandı. Sonuç olarak GnRH agonist tedavisine aday endometriozis olgula¬rında BMC ölçümünün ve tedavi boyunca izlenmesi¬nin gerekli olduğu kanısına vardık.

Abstract

In order to invastigate the impact of hypoestrogenism induced by GnRH agonists on BMC, 20 endometrio-sis patients have been observed for 1 year arid the results have been compared to that of Danazol group which have been analyzed retrospectively. In the group that triptorelin was administrated 3.75 mg/ 28 days, BMC was 0.0985 ± 0.033 glcm2 after 6 months of treatment while the baseline level was 1.080 ± 0.040 g/cm2. BMC was increased up to 1.015 ± 0.026 g/çm2 at the 6th month of cessation of treatment. Ca/creatinin ratio have been increasedduring the treatment and decreased after the cessa¬tion, whereas serum alkaline phospatase was still el¬evated. In the group that danazol was administrated 800 mg/day, 6 months, there was no significant BMC loss, although E2 levels were between early follicular range. It was duo to the androgenic effects of dana¬zol. We concluded that BMC screening is essential for GnRH agonist treatment candidates.
