Özet

Spondilolistezis, spinal stenozun en önemli sebeple¬rinden biri olup, şiddetli ağrı ile nörolojik defisit riskine sahiptir. Özellikle nöral kladikasyosu olan ağrılı has¬talarda cerrahi dekompresyon ve spinal füzyon en başarılı tedavi şeklidir. SSK Ankara Hastanesi, 1. ve 2. Ortopedi ve Travmatoloji Kliniklerinde, 2. - 5. derece arasında kayması olan, 25 spondilolistezisli hastaya, bu amaçla posterior dekompresyon, poste-rolateral ile "posterior interbody" (posterior cisimler arası) füzyon (PLIF) kombine olarak ve posterior ens-trümentasyon uygulandı ("Texas Scottish Rite Hospi¬tal" (TSRH) Sistemi : 14, "Compact Cotrel Dubousset" (C-CD): 9 ve ISOLA: 2). Hastalardan bi-rene "Novus Cage" ile diğerlerine trikortikal iliak greft-le PLIF uygulandı (Spondilopitozisi olan 2 hasta dışında). Preoperatif ortalama % 43.5 + 22.5 olan olistezis, postoperatif % 64.1 ± 36.8 ortalama korrek-siyon sağlanarak, ortalama % 23.0 ± 29.3'e getirildi. Ağrı ve fonksiyonel değerlendirme skalası ("Pain and Functional Assesment Scales": PFA) göre preopera¬tif 11.3 + 3.9 iken, postoperatif 1.8 ± 1.9'a geldiği be¬lirlendi. Tüm hastalarda solid füzyon elde edildiği saptandı. Bu verilerin ışığı altında spondilolistezisli hastalarda, posterior dekompresyon, posterolateral füzyon ve PLIF kombine edilerek, posterior enstrü-mentasyon uygulaması ile, yüksek karreksiyon ve kaynama oranlan ile başarılı sonuçlar elde edilebile¬ceği fikri elde edildi.

Abstract

Spondylolisthesis is one of the most important cause of spinal stenosis and characterised with severe pain and neurologic deficit risk. Especially for patients with neurogenic cladication, surgical decompression and spinal fusion is a good method of treatment. In the Orthopaedics and Traumatology clinics of Ankara Social Security Hospital, 25 patients with grade 2-5 spondylolisthesis were treated with posterior decompression, posterolateral and posterior interbody lumbar fusion (PLIF) and posterior instrumentation (TSRH : 14 patients, C-CD: 9 patients, ISOLA : 2 patients). PLIF was performed with Novus cage in one patient and three cortical autogenous iliac graft in the rest of the patients (Except 2 patients with spondylopitosis). In the preoperative period listhesis was 43.5 ± 22.5 % in our patient group and with 64.1 ±36.8 % correction rate it was 23.0 ± 29.3 % in the postoperative period. Pain and functional assesment scale (PFA) was 11.3 ± 3.9 preoperatively and 1.8 ± 1.9 in the postoperative period. All of the patients had solid fusion mass. In the light of these findings it's concluded that posterior decompression, posterolateral and posterior interbody lumbar fusion and posterior short segment fixation has high rates of fusion and succesfully appliciable to the patients with spondylolisthesis.
