Özet

Mandibulanın yüksek ve düşük hızlı ateşli silah yara¬lanmalarında erken dönemde açık redüksiyon ve rijid internal fiksasyon estetik ve fonksiyonel sonuçlar açısından yeni ufuklar açmıştır. Ancak kemik defekti bulunan yaralanmalarda sekonder kemik grefti öneril¬mektedir. Bu yaklaşımla kemik bütünlüğü ve yara¬lanma öncesi fasyal yapının sağlanması yumuşak doku kontraktürü gelişmeden önce başarılabilmek-tedir. Kasım - 1994 ve Mayıs 1996 tarihleri arasında Diyar¬bakır Asker Hastanesi'nde, mandibula ateşli silah yaralanması nedeniyle başvuran 13 hastaya erken dönemde açık redüksiyon, rigid internal fiksasyon ve primer yumuşak doku onarımı yapıldı. Başvuran 13 hastadan 8'inde mândibulada sadece bir yörede basit ya da parçalı fraktür mevcuttu. Bu hastalarda tedavi amacıyla yaralanma sonrası erken dönemde üst ve alt çeneye ark bar uygulaması ile interdental ilişki sağlandı. Eksternal yaklaşımla fraktür hatlarına mini-kompresyon plağı ile birlikte mini-plak ya da intermaksiller tespit yapılarak onarım sağlandı. 4 hastada ise rekonstrüksiyon plağı ile onarım gerek¬tiren çok parçalı fraktür ya da kemik defekti saptandı. 4 cm. kemik defekti bulunan bir hastaya sekonder-kemik grefti ile onarım yapıldı. Elde edilen estetik sonuçlar tatmin edici bulundu. Hastalarda postope-ratif dönemde ortalama ağız açıklığı 4 cm (3.2 - 5.1) saptandı. 4 hastada erken dönemde gelişen tükrük fistülü cerrahi girişim gerektirmedi. Bir hastada minimal anterior open-bite ile karşılaşıldı. Bu yazıda, mandibula ateşli silah yaralanması nede¬niyle öpere edilen 13 hastada elde edilen estetik ve fonksiyonel sonuçlar sunulmuştur. Erken dönemde açık redüksiyon, rigid internal fiksasyon ve yumuşak doku onarımı ile tedavi edilen hastalardaki sonuçlar literatürde bildirilen konvansiyonel ve sekonder giri-simlerle elde edilen sonuçlarla karşılaştırılmış ve tedavi yöntemleri gözden tartışılmıştır.

Abstract

Open reduction and rigid internal fixation add new dimensions to the aesthetic and functional results obtained from high and low velocity gunshot wounds of mandible. However, it is recommended secondary bone grafting for the treatment of bony defects of mandible. Structural bony integrity and pre-injury facial architecture can be restored in the absence of soft tissue contracture. In the period of November 1994 - May 1996, open reduction and internal fixation, and primer soft tissue repair were performed on 13 patients with gunshot wound of mandible at Diyarbakır Military Hospital. Eight patients had simple or comminuted fractures at only one region of mandible. In these patients, open reduction and internal fixation with mini-compression plate and mini-plate or intermaxillary fixation were performed at early post-traumatic period. Prior to the operation interdental fixation was performed in all patients. Reconstrruction plates were used for the treatment of patients with severe comminuted frac¬ture or bone defect of mandible. One patient with 4 cm. mandibular bone defect was treated by use of secondary bone graft. Aesthetic results were found to be satisfactory to the patients. Mouth opening in the postoperative period was found to be 3.2 - 5.1 cm (4 cm). It was observed minimal anterior open-bite in one patient. Salivary fistula in four patients required no surgical intervention in the postoperative period. In this paper, aesthetic and functional results of 13 patients who were operated for gunshot wound of mandible are presented. Results obtained with early bone and soft tissue reconstruction were compared conventional and secondary reconstrutcion modalities, and the treatment approaches are discussed.
