Özet

Haziran 1994 - Haziran 1996 tarihleri arasında GATA ÜrolojiABD'na infravezikal obstrüksiyon semptomları ile başvuran hastalardan benign prostat hiperplazili (BPH)112 tanesi bir yıl süre ile finasterid tedavisine alındı. Çeşitli sebeplerle tedaviye başlayamadığımız BPH'U20 hasta ise kontrol grubunu oluşturdu. Finasterid tedavisine alınan hastaların tedaviye baş¬lamadan önce, tedaviye başladıktan sonra 6. ve 12. aylarda prostat büyüklükleri, maksimum akım hızları, ortalama akım hızları, rezidüel idrarları, uluslararası prostat semptom skorları (I-PSS) ve prostat spesifik antijen (PSA) değerleri ölçüldü. Kontrol grubuna ise aynı parametreler ilk geldiklerinde 6. ve 12. aylarda bakıldı. Sonuçların istatistiksel olarak anlamlı olup ol¬madıkları tek yönlü vaıyans analizi yöntemi ile araştı¬rıldı. Parametre/erdeki değerlerin 6. ve 12. aydaki so¬nuçlarının istatistiksel olarak anlamlı değişikliğe uğradığı tespit edildi. Kontrol grubunda ise istatistik¬sel olarak anlamlı değerler elde edilmedi. Tedaviye alman hastaların prostat büyüklüklerinin ortalamaları 6. ayda % 7.29, 12. ayda % 16.34 oranında küçüldü¬ğü, maksimum akım hızlarının 6. ayda 1.268 ml/sn, W. ayda 2.015 ml/sn. arttığı, ortalama akım hızlarının 6. ayda 0.767 ml/sn., 12. ayda 1.407 ml/sn. arttığı, rezidüel idrarlarının 6. ayda % 34.756, 12. ayda ise % 61.011 oranında azaldığı, I-PSS değerlerinin ise 6. ayda 2.504, 12. ayda ise 4.333 azaldığı, PSA değer¬lerinin 6. ayda % 33.613, 12. ayda ise % 55.48 ora-Rtnda azaldığı tespit edildi. Bu sonuçlara göre finasterid tedavisine alman BPH'li hastaların tedaviden fayda gördükleri ve ilacın BPH tedavsinde gold standard olan transüretral prostat re-zeksiyoriu (TUR-P) hariç, birçok tedaviye alternatif olabileceği görülmüştür.

Abstract

among patients suffering BPH who applied to the Urology Department of GMMA with symptoms of infravesical obstruction "between June 1994 and June 1996, 112 took finasteride treatment for a year. 20 patients with BPH for whom we couldnt start any therapy for various reasons formed the control group. Prostate sizes of patients under finasteride treatment, peak flow rate, mean flow rate, residual urine I-PSS and PSA values were measured before the treatment and at the 6th and 12th months of the therapy. On the other hand, the control group was checked for the same parameters at their first admission and at the 6th and 12th months of the therapy. Whether the results make any sense statistically was checked by the method of one way variance analysis. It was found out that the results of the parametrical values at the 6th and 12th months underwent significant changes. No statistically significant changes were obtained in the control group. It was observed that the average prostate sizes of the patients under treatment got smaller by 7.29% at the 6th month and 16.34% at the 12th. Their peak flow rate increased by 1.268 ml/sec at the 6th month and 2.015 ml/sec at the 12th and their mean flow rate increased by 0.767 ml/sec at the 6th month and 1.407 ml/sec at the 12th. Meanwhile, residual urine decreased by 34.756% at the 6th month and 61.011% at the 12th and I-PSS values decreased 2.504 at the 6th month and 4.333 at the 12th. Finally PSA values decreased by 33.613% at the 6th month and 55.48% at the 12th. According to these results, it was seen that the BPH patients, who undertook therapy with finasterid, benefited from the therapy and the drug might be an alternative for several therapies except TUR-P which is the gold standart for BPH treatment.
