Intranazal eroin kullanımı ve uzun süre immobilizasyon sonucu gelişmiş bir rabdomyolizis olgusu

Özet
Adale iskemisi ve travması, sıcak çarpması, eroin-kokain aşırı dozu gibi nedenler ile oluşan rabdomyoli-zisde myoglobinürik akut böbrek yetmezliği oluşabil-mektedir. Tek başlarına nefrotoksik olmayan hemog¬lobin ve myoglobin dehidratasyon, asidoz gibi komorbid faktörlerin etkisi ile böbrek yetmezliğine ne¬den olabilmektedir. Literatürde, intravenöz eroin kul¬lanımına bağlı bilinç kaybı, uzun süre aynı postürde kalma ve hiperpireksi sonucu gelişen rabdomyolizis olguları bildirilmektedir. Burada inhalasyon yolu ile eroin kullanımından hemen sonra banyoya giren ve senkop geçirerek 12 saat boyunca aynı postürde kal¬dıktan sonra yakınları tarafından bulunarak hastane¬ye getirilen 24 yaşında bir erkek hasta sunulmakta¬dır. İlk muayenesinde tüm vücutta yaygın ekimozlar, sağ bacakta ve skrotumda ödem, sağ cruriste kom-partman sendromu ve sağ peroneal sinir paralizisi saptanmıştır. Laboratuvar tetkiklerinde lökosit, üre, kreatinin, AST, ALT, kreatin kinaz (CK), LDH, idrarda myoglobin düzeyleri yüksek bulundu. Hastaya Orto¬pedi servisi tarafından acil fasiotomi yapıldı, oligürik seyretmekte olan hastada metabolik asidoz gelişme¬si üzerine 4 kez hemodiyaliz uygulandı, yatırılışının 11 nci gününde poliürik dönem başladı ve 25 gün de¬vam etti. Böbrek işlevleri ile CK düzeylerinin normale dönmesini takiben, hasta ortopedi kliniğine nakledildi ve irreversibl sağ peroneal sinir paralizisi nedeniyle gelişen "düşük ayak" için artrodez ameliyat yapıldı. Olgunun eroin inhalasyonuna bağlı nadir bir örnek ve yüksek düzeyde CK değerlerine sahip olması nede¬niyle, sunulmasına karar verildi.
Abstract
The myoglobinuric acute renal failure may be seen in rhabdomyolysis due to muscle ischemia and injury, heat stroke and heroin-cocaine abuse. Hemoglobine (Hb) and myoglobine (Mb) which are not nephro-toxic chemicals, may cause renal failure with the presence of some co-morbid conditions such as dehydration and acidosis. Unconsciousness, prolonged resting in the same posture and hyperpyrexia may cause rhabdomyolysis. Herein, we present a 24-year-old male, found in a fainted state by his parents in the shower room after taking intranasal heroin 12 hours ago. On the first physical examination, recorded the massive ecchymoses on whole body, edema on his right leg and scrotum, right peroneal nerve paralysis due to right crural compartment syndrome. On laboratory investigation WBC, urea, creatinine, AST, ALT, creatine kinase (CK), LDH, urinary Mb levels were found higher than normal. The operation of fasciotomy was performed on his right leg by an orthopaedist. The urinary output did not reach to a non-oliguric level and four hemodialysis sessions were scheduled on alternate days, for metabolic acidosis. On the eleventh day of hospitalization, began the polyuric phase and continued for 25 days. After normalization of renal functions and CK levels, he was discharged to department of orthopaedics and an arthrodesis operation was performed for the "drop foot" due to the irreversible right peroneal nerve paralysis. It was decided to report the case for the reasons of a rare example of a rhabdomyolsis due to heroin inhalation and the high level of CK.
