Özet

Kronik renal yetmezlikti hastalarda renal osteodistro-fiyi saptamada invaziv olmayan yöntemlerin değe¬rinin araştırıldığı bu çalışmada henüz sürekli diyaliz programına alınmamış 20 kronik renal yetmezlikti erkek hastada transiliyak kemik biyopsisi uygu¬lanmış, serum kalsiyum, fosfor, 1otal alkalen fosfataz, parathormon ve alüminyum seviyeleri ölçülmüş, pelvis, el ve klavikula grafileri çekilmiştir. Biyopsi sonuçlarına göre 6 hastada (% 30) osteitis fibroza, 1 hastaa (% 5) hafif üremik kemik hastalığı ve 1 hastada (% 5) ise mikst üremik kemik hastalığı saptanmıştır. Biyopside renal osteodistrofi saptanan 8 hasta (A) ile saptanmayan 12 hasta (B) iki grup halinde ayrılmış, gruplar arasında serum kalsiyum, fosfor ve total alkalen fosfataz seviyeleri bakımından fark bulunamamıştır. Serum parathormon seviyeleri A grubunda, serum alüminyum seviyeleri ise B grubunda diğer gruba göre yüksek bulunmuştur. Radyografik incelemeler arasında ise en yüksek sensitivite (% 87.5), spesifisıte (% 33) ve pozitif tahmin değeri (% 46) pelvis grafisinde bulunmuştur.

Abstract

Twenty male nondialyzed chronic renal failure pati¬ents were included into the study to investigate the value of noninvasive methods in diagnosis of renal osteodystrophy. Transiliac bone biopsy was performed in all patients. Serum calcium, phosp¬horus, total alkaline phosphatase, parathyroid hormone and aluminum levels were determined. Pelvis, hand and clavicle plain radiographs were also obtained. On the examination of bone biopsy speci¬mens, osteitis fibrosa in 6 patients (% 30), mild uremh bone disease in 1 patient (% S) and mixt uremk bone disease in 1 patient (% 5) were detected. Patients were groupped as A (8 patients with renal osteodystrophy findings on bone biposy) and B (12 patients without renal osteodystrohhy findings on bone biopsy). Serum calcium, phosp¬horus and total alkaline phosphatase levels did not show difference between two groups. However, serum parathyroid hormone levels in group A and serum aluminum levels in group B were significantly higher compared to other group. Among the radiog-raphic examinations pelvis radiographs had highest sensitivity (% 87.5) spesificity (% 33) and positive predictive value (% 46).
