Özet

ilk kez Vater tarafından tanımlanan ve Alonzo-Lej ile arkadaşları tarafından sınıflandırılan koledok kistleri, karaciğer içi ve dışı safra yollarının doğumsal kistik genişlemeleridir. Koledok kanalının fuziform kistik genişlemesi şeklinde olan Tip l koledok kisti, koledok kisti olgularının % 80'ini oluşturur. Erişkin koledok kistlerinin kolanjü sekonder bilier siroz ve karsinoma gelişimi gibi riskleri vardır. Karın ağrısı, sarılık ve karında kitle yakınması ile kliniğimize başvuran bir hastada dev Tip l koledok kistinin bulunduğu belir-lendi. Olgu operasyona alındı ve yaklaşık 1-1,5 litre hacimli kiste kistotomi uygulandıktan sonra proksimal bölüme total eksizyon uygulandı. Distal bölüm ise yoğun vasküler yapı nedeni ile Lilly yönteminde tanımlandığı gibi intramural eksize edildi ve operasyon Roux-en-Y hepatiko jejunostomi ile tamamlandı. Postoperatif dönemde bir komplikasyon olmaksızın hasta taburcu edildi. Büyük kistlerde ileri derecede vasküler oluşum nedeni ile total eksiz-yonun güç olduğu olgularda intramural mukozektominin iyi bir cerrahi seçenek olduğu vurgulandı

Abstract

Choledochal cysts, first defined by Vater and classified by Alonzo-Lej, are congenital cystic dilatation of intrahepatic and extrahepatic biliary tracts. Type I choledochal cysts, the fusiform cystic dilatation of the choledochal duct, compromises 80 % of all choledocal cystic diseases. A choledochal cysts has risks of development of cholangitis, secondary biliary cirrhosis and carcinoma. A giant Type I choledochal cyst was determined on a patient who has admitted to our clinics with the symptoms of abdominal pain, jaundice and a mass in the abdomen. The case was undertaken to operation. At the operation after applying cystostomy to the cyst which is 1-1.5 It in volume, total excision was applied to proximal region. Because of the condensed vascular structure, distal part was intramurally excised as described by Lilly Method and then operation was completed by Roux-en-Y hepaticojejunostomy. Patient was discharged with no complication in the post-operative period. It is uppointed that, intramural mucosectomy is a good surgical choice in cases 'where total excision is difficult because of high level vascularity in large cysts.
