Düşük akım kapalı sistem anestezinin idrar keton cisimleri düzeyine etkisi

Özet

Düşük akımlı kapalı sistem anestezi uygulanan olgularda idrar keton cisimleri düzeyinde oluşan değişiklikler ve wash out (yüksek taze gaz akımı ile yıkama) uygulamasının buna etkisi araştırıldı. Çalışmaya periferik sinir lezyonu nedeniyle greftleme uygulanan 60 olgu (ASA l ve II) alındı. Rastgele 2 gruba ayrılan olgulardan, Grup-l'e (n=30) düşük akım kapalı sistem anestezi, Grup-lt' ye (n=30) yüksek akım yarı kapalı sistem anestezi uygulandı. Her 2 grup olgularda; elektrokardiyografi, arteryel kan basıncı, arteryel oksijen saturasyonu, inspiratuar ve end tidal oksijen, karbondioksit, nitroz oksit ve volatil anestezik konsantrasyonları sürekli olarak izlendi. Ayrıca tüm olgulardan; indüksiyondan önce, indüksiyondan 30 dk sonra, intraoperatif 2 saatte bir, düşük akım kapalı sistem anestezi sonlandırıldığında ve 20 dk wash out uygulamasını takiben idrar örnekleri alındı ve keton cisimleri düzeyleri saptandı. Grup-l olgularda; idrar keton cisimlerinde, düşük akıma geçtikten sonra progresif artış, tekrar yüksek akıma geçiş ve wash out uygulaması ile de.belirgin azalma saptanırken, Grup-ll olgularda önemli bir değişiklik izlenmedi. Grup-l olgularda vücut ısısının operasyon süresince korunduğu, buna karşın Grup-ll olgularda progresif olarak düştüğü saptandı. Ayrıca, Grup-l'de inspiratuar ve end-tidal nitroz oksit ve inspiratuar nitrojen değerlerinde progresif artış, oksijen değerlerinde ise azalma izlendi. Grup-ll'de bu değerlerde anlamlı bir değişiklik saptanmadı. Sonuç olarak, düşük akım kapalı sistem anestezi uygulamalarında idrar keton cisimlerinde progresif bir artış izlenmektedir. Operasyonun bitiminden 20 dk önce yüksek akıma geçiş ve wash out uygulaması ile idrar keton cisimlerinde belirgin azalma oluşmaktadır.

Abstract

The differences of urine ketone bodies in patients with low flow closed system anesthesia and the effect of wash out procedure to these were investigated. Sixty patients (ASA I and II) undergone peripheral nerve grafting were studied. The patients were randomized into 2 groups, as Group I (n=30) low flow closed system anesthesia, Group*!I (n=30) high flow anesthesia. In both groups electrocardiography, arterial blood pressure, arterial oxygen saturation, inspiratory and end tidal oxygen, carbondioxide, nitrous oxide and volatile anesthetic concentrations were screened. Before induction, 30 minutes after induction, every 2 hours intraoperatively, after low flow closed system was ended and after wash out procedure for 20 minutes urine samples were collected and detected ketone body levels. In Group I, an increase in urine ketune bodies was observed progressively, when high flow was re-used and by wash out procedure, a significant decrease was observed. However in group II patients no significant change was founded. During the operation in Group I patients the body temperature was observed to be constant, however decrease in Group II patients. In Group I patients inspiratory and endtidal nitrous oxide and inspiratory nitrogen have increased progressively, while oxygen decrease. In Group II patients no significant change was observed. In conclusion, in low flow closed system anesthesia urine ketone bodies progressively increase. Urine ketone bodies decrease significantly if high flow and wash out procedure is begun 20 minutes before end of the operation.
