Stres inkontinansın cerrahi tedavisinde Burch operasyonu ile kolporafi anterior-Kelly plikasyonun karşılaştırılması

Özet
Stres inkontinans ve sistosel sıklıkla birlikte görülen Minik patolojilerden olup, hasta için hem sosyal, hem de hijyenik bir problem olmaktadır. Bu amaçla eforla idrar kaçırma şikayeti ile başvuran yaşları 31-69, pariteleri 2-9 arasında değişen ve gerçek inkontinans tanısı alan 40 olgu çalışma grubu olarak alındı. Bu 40 olgu, uygulanan Burch operasyonu veya Kolporafi anterior Kelly plikasyonu operasyonuna göre herbiri 20'şer kişiden oluşan iki gruba ayrıldı. Her bir gruptaki başarılı yada başarısız olguların sonuçlan ile karşılaştırıldı. Altı aylık izlem sonunda gerek semptom, gerekse de bulgular yönünden Burch operasyonu uygulanan grupta başarı oranı % 64 iken, Kolporafi anterior-Kelly plikasyonu uygulanan grupta ise % 60'lık bir başarı oranı saptandı. Burch operasyonu uygulanan grupta başarı oranı daha yüksek bulunmasına-rağmen, bu iki operasyon arasındaki fark istatistiksel olarak anlamlı değildi. Bu nedenle tekniğin daha kolay uygulanabilir olması, morbiditenin daha az olması, operasyon, hospitalizasyon ve kateterizasyon sürelerinin daha kısa olması ve eşlik eden sistoselin varlığında, sistoselin de düzeltilmesine olanak sağlaması nedeni ile vajinal yol ilk tercih edilecek yöntem olarak görülmektedir. Ayrıca preoperatif dönemde stres inkontinanslı hastalara iyi bir değerlendirme yapılması, operasyon öncesi uygun olgu seçimi ve uygun ameliyat tekniğinin saptanması başarı oranını arttıracaktır.
Abstract
Strese incontinence and cystocele, which are pathologîcal conditions commonly seen together, present themselves as social and hygienic problems. To investigate different therapy modalities, 40 patients with genuine urinary stress incontinence were accepted as the study group. Patients were divided into two groups having 20 patients each according to the preference of the performed operation as Burch colposuspension or colporraphy anterior-Kelly urethral plication. In each group the results of successful or unsuccessful cases were compared with each other and with the results of other anti-stress incontinence operations. After 6 months of follow-up success rate, based on symptoms and findings, was 80% in the group of patients having Burch operation, whereas the success rate was 60% in the other group in which the anterior colporraphy-Kelly plication procedures were perfor-med. Although the success rate was higher in the Burch group, no statistical significance was found between these two methods. Therefore, vaginal procedures appears to be the method of preference because of its easy performance, less morbidity, less operation-hospitalisation-catheterisation times and possibility of repairing commonly associated cystocele at the same session. Moreover, a complete and detailed preoperative evaluation, appropriate patient selection and choice of most favourable operational technique will possibly increase'lhe rate of success.
