Özet

Amaç: Tip 2 diyabetli hastalarda random idrarda ölçülen albumin/kreatinin oranı ile diyabetik retinopati arasındaki ilişkiyi incelemek ve en iyi 'A/K (Albumin/Kreatinin) eşik değerini saptamak.Materyal ve Metod: Türk Diyabet Cemiyeti Diyabet Hastanesinde Tip 2 diyabetli 94 hastadan alınan random idrarda Bayer DCA 2000 Albumin/Creatinine Analiser ile A/K oranı saptandı ve retina uzmanı tarafından hastaların fundus muayeneleri yapıldı. Birçok A/K eşik değerinde (5-100 mg/g arasında) duyarlılyık, özgüllük, yalancı pozitif ve negatif oranlar ile yalancı pozitif ve negatif öngörü değerleri saptandı.Bulgular: Diyabetik retinopatiyi öngörmede en iyi 'A/K eşik' değerleri 10 ve 12 mg/g olarak bulundu. 10 mg/g en yüksek pozitif (% 72,9) ve 12 mg/g en iyi negatif (% 66,7) öngörü değeri olarak saptandı. 10 mg/g alındığında retinopatiyi öngörmede en iyi duyarlılık ve özgüllük değerleri sırasıyla % 75 ve % 61,9 olarak bulundu. 12 mg/g değerinde duyarlılık ve öz-güllük sırasıyla % 67,3 ve % 69,1 olarak bulundu.Sonuç: Nispeten ucuz, kolay ve noninvazif bir yöntem olan random idrarda A/K saptanması kabul edilebilir bir doğruluk ve öngörü ile diyabetik retinopati derecesini tahminde kullanılabilir.

Abstract

To investigate the correlation between albumin/creatinine (A/C) ratio in random urine samples with the retinopathy status in type 2 diabetics and to determine the best "A/C cutoff " for identifying those patients with retinopathy.A/C ratio was measured in random urine samples by using Bayer DCA 2000 Albumin/Creatinine Analyser® and fundus examination was performed by a retinal specialist in 94 type 2 diabetic patients at a foundation hospital specialized in diabetes care. Sensitivity, specificity, false positive and negative rates with positive and negative predictive values of several A/C cutoffs (between 5 and 100 mg/g) was calculated.10 and 12 mg/g were found to be as the most useful A/C cutoffs for prediction of diabetic retinopathy. "10 mg/g" yielded the highest positive (72.9 %), while "12 mg/g" gave the best negative (66.7 %) predictive values. Sensitivity and specificity for prediction of retinopathy were calculated as "75 and 61.9 %" respectively, if "10 mg/g" was selected. Sensitivity and specificity would be "67.3 and 69.1 %" respectively, for a cutoff of "12 mg/g".This relatively simple, noninvasive and inexpensive laboratory test of A/C ratio in random urine would be useful for assessing diabetic retinopathy status with reasonable accuracy and precision.
