Özet

Karaciğer kist hidatiği Echinococcus granulosus parazitinin neden olduğu ilerleyici karakterde benign bir hastalık olup yüksek morbidite ve mortaliteyle seyreden kist enfeksiyonu, etraf dokulara bası ve kistin çevre yapılar içerisine rüptürü gibi komplikasyonlara sebep olabilir. Nadiren kistin travmatik veya kendiliğinden rüptürüyle anafilaktik reaksiyon gelişebilir ve ölümle sonuçlanabilir. Anafilaksiye bağlı morbidite ve mortalitenin önlenmesinde zamanında konulan tanı ve uygulanan tedavi bu nedenle çok önemlidir. Kist içeriğinin travma olmaksızın kan dolaşımına geçmesine bağlı anafilaksi gelişmesi oldukça nadir görülür. Eğer cerrahi tedavi uygulanmazsa kist rüptürü ölümle sonuçlanabilir.Bu olgu sunumunda, anafilaksi ayırıcı tanıları arasında hidatik hastalığın da düşünülmesi gerektiğini vurgulamak için ürtiker ve solunum sıkıntısı ile hastanemiz acil servisine başvuran ve karaciğer kist hidatik rüptürü tanısıyla opere edilen 28 yaşında erkek hasta sunulmaktadır.

Abstract

The hepatic hydatid cyst is a progressive type of benign disease caused by Ecinococcus granulosus and may cause such complications that result in serious morbidity and mortality including suppuration of cyst, compression of surrounding tissues and rupture into adjacent structures. Rarely anaphylactic reactions due to traumatic or spontaneous rupture of hepatic hydatid cyst may occur and death ensue. Therefore, timely diagnosis and treatment are very important to prevent the morbidity and mortality associated with the anaphylaxis. Nontraumatic leakage of cyst contents into the blood circulation is an uncommon triggering factor for anaphylaxis. Unless treated surgicaly, rupture can result in death. In this case report, we present a 28 year old male patient in order to emphasize the hydatid disease in the differential diagnosis of anaphylaxis who was admitted to the emergency room with urticaria and respiratory distress and operated for ruptured hepatic hydatid cyst.
