Sirozlu hastalarımızda hepatik ensefalopati’yi tetikleyici faktörler

Özet
Amaç: Hepatik ensefalopati sirotik hastalarda sık görülebilenve ölümcül olabilen bir komplikasyondur. Bu yüzden erken teş- hiş ve etyolojiye yönelik tedavi hayati önem taşımaktadır.Gereç ve Yöntem: İstanbul Göztepe Eğitim ve Araştırma Has- tanesi Gastroenteroloji Kliniği’nde 2005-2007 yılları arasındaHepatik Ensefalopati (HE) tanısıyla yatırılarak tedavi edilen81 olguda, ensefalopatiyi presipite edici faktörleri retrospektifolarak araştırdık. Presipite edici neden olarak; enfeksiyonlar,konstipasyon, elektrolit imbalansı, gastrointestinal kanama,dehidratasyon, hepatocellüler karsinom (HCC), ilaç-alkolintoksikasyonu, HCC dışı malignite ve fazla protein alımıaraştırıldı.Bulgular: Hastalarımızda HE’yi tetikleyen nedenler sıklıksırasına göre % 33 enfeksiyonlar, % 25 konstipasyon ve % 12elektrolit imbalansı olarak saptandı. Enfeksiyonlar içinde iseen sık neden spontan asit enfeksiyonu saptandı (36 %). Bulgu- larımız daha önce yapılan klinik çalışmalarla benzerlik göste- riyordu.Tartışma: Sirozlu hastalarda HE gelişiminin önlenmesi vetedavisi için, enfeksiyon ve konstipasyonla mücadele anahtarrol oynamaktadır.
Abstract
Aim: Hepatic encephalopathy is a frequent complication ofcirrhosis. This complication may culminate in death thereforeearly diagnosis and prompt treatment is essential.Materials and Methods: In this study, we retrospectively eva- luated the predisposing factors in eighty one hepatic encepha- lopathy patients who were admitted to Gastroenterology wardat Istanbul Goztepe Teaching and Research Hospital in betwe- en 2005 and 2007. Case notes of the patients were reviewedand if present; any infections, constipation, electrolyte imba- lance, gastrointestinal bleeding, dehydration or hepatocellularcarcinoma were noted.Results: Thirty three percent of patients had an infection, 25%had constipation and 12% had electrolyte imbalance. The mostcommon type of infection was spontaneous ascites infection(36% of the all infections). These results agreed with thosereported in the previous clinical studies.Conclusions: Struggle against infections and constipation ispivotal for prevention and management of hepatic encephalo- pathy in cirrhotic patients.
