Radikal prostatektomi spesmenlerindeki MMP-2’nin, MMP-9’un ve TIMP-1’in immünohistokimyasal ekspresyonlarının prognostik değeri

Özet
Amaç: Prostat biyopsilerinde saptanan tümör grade (Gleson Skor) ve preoperatif prostatik spesifik antijen (PSA) değerleri, retropubik radikal prostatektomi (RRP) sonrası hastalığın gidişini doğru olarak tahmin etmede yetersiz kalmaktadır. Daha önce yapılan çalışmalarda, prostat kanserlerinde, matriks metalloproteinaz (MMP) ve doku metalloproteinaz inhibitöy (TIMP) salınımının arttığı bildirilmiştir. Bu çalışmadaki amaç radikal prostatektomi materyallerinde MMP2, MMP9 ve TIMP1'in prognostik önemini saptamaktır. Gereç ve Yöntem: RRP operasyonu uygulanmış 66 hastanın klinik ve patolojik bulguları retrospektif olarak gözden geçirildi. Formalinle fiske edilmiş doku kesitleri MMP2, MMP9 ve TIMP1 antikorları kullanılarak manuel metod ile immünhistokimyasal olarak boyandı. Pozitif kontrol olarak çevre benign prostat dokusu kullanıldı. İmmünreaktiviteyi değerlendirirken, boyanmanın yaygınlığı ve şiddeti dikkate alındı. Sonuçlar yaş, preoperatif PSA değerleri, TRUS skoru, RRP Gleason skoru, tümör volümü, sinir, damar, kapsül ve seminal vezikül tutulumu, ekstrakapsüler yayılım, cerrahi sınır pozitifliği, lenf nodu metastazı gibi klinik ve patolojik bulgularla karşılaştırıldı. Bulgular: Altmış altı olgunun altmış ikisinden MMP2 ve MMP9, on birinde TIMP1 salınımı izlendi. MMP2 ve TIMP1 salınımı bu çalışmaya alınan prognostik kriterlerin hiçbirisiyle korelasyon göstermedi. MMP9 da korelasyon göstermemekle beraber MMP9 dağılımı ile tümör volümü (P=0.052) ve MMP9 şiddeti ile lenf nodu metastazı varlığı arasında (P=0.075) anlamlılığa yakın derecede fark gözlendi. Sonuç: MMP2, MMP9 ve TIMP1 salınımı ile klinik ve patolojik faktörler arasında korelasyon izlenmemiştir. Ancak daha büyük serilerle çalışma yapılması, imünhistokimyasal değerlendirmenin imaj analiz yöntemi ile değerlendirilmesi RRP sonrası bu biyobelirteçlerin prognostik değerini daha net ortaya koyabilir
Abstract
Introduction: Tumor grade (Gleason score) detected in histopat- hological analysis of biopsy and surgical specimens, and pre- operative prostate specific antigen level fail to predict diseaseprogression after radical retropubic prostatectomy (RRP). Previous studies documented high levels of matrix metalloprote- inase (MMPs) in metastatic prostate cancer. Our aim was to de- termine prognostic value of MMP2, MMP9 and TIMP1 detected histopathologically in radical prostatectomy specimens. Materials and Methods: Clinical and pathological data of 66 patients with RRP were retrospectively reviewed. Formalinfixed tissue sections were immunostained by a manual method using MMP2, MMP9, and TIMP1 antibodies. Peripheral be- nign prostatic epithelium contained in the removed specimen was used as a positive control. Immunoreactivity was evalu- ated in consideration of the diffusiveness, and strength of the staining. Results were compared with clinicopathological characteristics such as patient’s age, preoperative PSA level, TRUS&Gleason score, tumor volume, extracapsular extension, surgical margin, invasion of capsule, seminal vesicle, neural and vascular, and lymph node metastasis. Results: Specimens out of 66 cases stained positively with MMP2 and MMP9, and in 11 cases TIMP1 expression was ob- served. Expression of MMP2 and TIMP1 did not correlate with any prognostic parameter analyzed in this study. MMP9 did not correlate with any of the parameters. However a nearly statis- tically significant difference was observed between distribution of MMP9 staining and tumor volume (P=0.052) and also bet- ween intensity of MMP9 staining and the presence of lymph node metastasis (P=0.075). Conclusion: Although these data do not show any correlation between expressions of MMP2, MMP9 and TIMP1 and clinicpathologic prognostic factors, further studies in larger series and using image analysis in the evaluation of immunostaining are required to establish prognostic value of these biomarkers after RRP.
