Nöroloji kliniğinde yatarak tedavi gören geriatrik hasta grubunun özellikleri

Özet
Geriatrik yaş grubundaki hastalarda nörolojik sorunlara oldukça sık olarak rastlanır. Bu nedenle de, nöroloji kliniklerinde yatarak tedavi gören hastaların çoğunun geriatrik yaş grubunda olduğu kanısı hakimdir. Çalışmanın amacı bu kanının doğruluğunu araştırmak ve geriatrik hasta grubundaki nörolojik tanıların dağılımını ve mortalite oranını saptamaktır. Ankara Tıp Fakültesi Nöroloji Anabilim Dalı'nda 1997 yılında yatarak tedavi gören hastalar retrospektif olarak incelenmiş ve 65 yaş ve üzeri geriatrik yaş grubu ("yaşlı"), 64 yaş ve altı "yaşlı olmayan" grup olarak kabul edilmiştir. Kliniğimizde 1997 yılında yatarak tedavi gören toplam 1000 hastanın sadece %32'si ise 65 yaş ve üstünde olup "yaşlı" gruba girmektedir. Gruplar istatistiki olarak nörolojik tanı açısından karşılaştırıldıklarında iskemik (%57.2) ve hemorajik (%7.5) serebrovasküler olaylar ve demans (%4.4) "yaşlı" grupta, santral sinir sisteminin demiyelinizan hastalıkları (%10.4), nöromüsküler bileşke hastalıkları (%3.8), epilepsi (%3.8), başağrısı (%3.1), ataksi (%1.8) ve "diğer" başlığı altındaki hastalıklar "yaşlı olmayan" grupta anlamlı olarak daha fazladır. "Yaşlı" grupta vefat oranı "yaşlı olmayan" gruba göre iki kat daha fazladır. Sonuçta, Nöroloji Anabilim Dalı'nda yatarak tedavi gören hastaların çoğunun genel kanının aksine geriatrik yaş grubunda olmadığı, Nöroloji Anabilim Dalı'nda yatarak tetkik ve tedaviyi gerektiren hastalıkların yaş gruplarına göre belirgin farklılıklar gösterdiği ve nörolojik bir hastalığa bağlı olarak vefat oranının geriatrik yaş grubunda daha fazla olduğu görülmüştür.
Abstract
Neurological problems are frequently encountered in the geriatric population. Thus, there is a wide spread belief that the patients in the neurology clinics mostly belong to the geriatric population. The aim of the present study is to investigate the validity of this belief and to ascertain the distribution of the neurological diagnoses and the mortality rate in the geriatric patients. The hospitalized patients who were treated in 1997 in the Neurology-Department of the Ankara Medical School are investigated retrospectively and the patients who were 65-year-old or older are grouped as "old" and who were 64-year-old or yonger are grouped as "not old". From the 1000 patients who were treated in 1997 in our department, only 32% are 65-year-old or older. When the groups are compared statistically for the distributions of the neurological diagnoses, ischémie (57.2%) and haemorrhagic (7.5%) cerebro-vascular accidents and dementia (4.4%) are more frequent in the "old" patients, whereas demyelinating diseases of the central nervous system (10.4%), diseases of the neuromus-cular junction (3.8%), epilepsy (3.8%), headache (3.1%), ataxia (1.8%) and disorders under the "others" heading are more frequent in the "not old" patients. The mortality rate of the "old" group is twice that of the "not old" group. As the result, contrary to the widespread belief that most of the neurological hospitalized patients do not belong to the geriatric age group; the distribution of the diagnoses of the neurological diseases differs according to the age group and the rate of mortality secondary to a neurological disease is higher in the geriatric age group.
