Akut ekstremite iskemilerinde geriatrik olguların cerrahi tedavi sonuçlarının genç olgularla karşılaştırılması

Özet

1991 ile 1997 yılları arasında toplam 104 olguya 128 kez tromboem-bolektomi uygulandı. Bu olgulardan 68'i geriatrik grupta yer almaktaydı. Olguların 38'i kadın, 66'sı erkekti. 22 olguda üst ekstremite, 82 olguda ise alt ekstremitede akut iskemi bulguları vardı. Yaş ortalaması geriatrik grupta 67.3, diğer grupta 52.6 idi. Olguların hepsine lokal anestezi ile operasyon uygulandı. Dört olguya dört, üç olguya üç, 8 olguya iki kez tromboembolektomi uygulandı. Sekiz hastaya amputasyon gerekti. Bu olguların beşi geriatrik, üçü diğer grupta yer alıyordu. Beş olgu erken dönemde kaybedildi ve bu olguların hepsi geriatrik gruptaydı. Amputasyon yapılan olgulardan biri exitus oldu. Akut oklüzyon nedenleri irdelendiğinde; iskemik kökenli atrial fibrilasyon geriatrik olgularda %55.8 (38 olgu), diğer grupta %16.6 (6 olgu) oranında görüldü. Kapak hastalığına bağlı atrial fibrilasyon geriatrik olguların %16 (11olgu)'sında, diğer grubun %25 (9 olgu) inde gözlendi. İki olguda mitral pozisyonda prostetik kapak vardı. Bir olguda sol atriumda miksoma saptandı. Sekiz olguda aterosklerotik damar yüzeyinde tromboza bağlı tıkanma saptanırken, 19 olguda herhangi bir neden bulunamadı. Fischer's exact test kullanılarak yapılan değerlendirmede, iki grup arasında mortalite ve amputas-yon oranları açısından istatistiksel olarak fark bulunamadı. Fakat %7.5 oranındaki mortalitenin yaşlı grupta olması olgu sayısının arttığı durumlarda istatistiksel açıdan anlamlı sonuçlar çıkacağını düşündürmektedir. Yapılan değerlendirmede mortaliteyi belirleyen en önemli nedenin hastanın preoperatif genel durumu olduğu saptandı. Yaşlı olgularda özellikle kardiak ve renal fonksiyonların genç gruba göre kötü olması, mortalitenin bu grupta daha yüksek olmasının nedeni olarak görüldü. Amputasyon oranlarında iki grup arasında fark bulunamadı. Amputasyonu etkileyen en önemli faktörün iskemide kalınan süre olduğu belirlendi.

Abstract

For the comparison of the results of the surgical correction of acute extremity ischemia in the young and geriatric patients; 128 tromboembolectomy were performed in 104 cases between 1991 and 1999. 68 patients were in the geriatric and 36 cases were in the younger age group (38 males, 66 females). Acut ischemia was at upper extremities in 22, and at the lower extremites in 82 cases. Mean age was 68.5 in the geriatric, and 54.5 in the younger group. All cases were operated with local anestesia. Amputation were performed in 8 cases. 5 cases had died in the early postoperative period, and all of them were in the geriatric group. 38 patients in geriatric and 6 patients in the younger group had atrial fibrillation due to ischemic heart disease, 11 patients in the geriatric group and 9 patients in the younger group had atrial fibrillation due to valvular heart disease. 2 patients had mitral valve replacement. Acute thrombotic occlusion at the cite of atherosclerotic plauque was observed in 8 cases and neither any predisposition nor any cause was found in 19 patients. Fisher's exact test was applied and no statistical significance was found between the mortality and amputation rates of the two groups. But as all the mortality was in the geriatric group it seems that there will be a statistical significance if the study will be carried out in larger series. The cardiac and renal disorders that may occur in higher rates in the geriatric group play an important role in the mortality. The most important factor that may increase the amputation rate is the duration of ischemia
