Geriatrik hastalarda safenektomi öncesi doppler ultrasonografi'nin önemi

Özet
Yaşlı hastalarda venöz valvlerde yetmezlik ve dejenerasyona bağlı venöz dolaşım fonksiyonları bozulur. Bu olgularda safenektomi yapılırsa alt ekstremitede belirgin ödem gelişebilir. Doppler ultrasonografi (USG) alt ekstremite venöz sistemin değerlendirilmesinde son derece etkili bir yöntemdir. Derin venlerin durumu, valv yetmezliği, venöz reflü doppler USG ile son derece iyi değerlendirilebilmektedir. Biz bu çalışmada koroner bypass operasyonuna alınan olgularda preoperatif doppler USG ile normal venöz fonksiyon tesbit edilen olgularda safenektomi sonrası alt ekstremite ödemi gelişip gelişmediğini tesbit etmeyi amaçladık. Çalışmaya koroner bypass operasyonu yapılan ve safen ven grefti çıkarılan olgular alındı. Tüm olgulara preoperatif ve postoperatif 1. ayda her iki alt ekstremiteye doppler ultrasonografi (USG) yapıldı. Doppler USG ile venöz ve arteryel akım hızları ölçüldü, venöz sistem tromboz, reflü ve valv yetmezliği açısından değerlendirildi. Doppler USG ile normal venöz fonksiyona sahip olduğu tesbit edilen olgularda safenektomi sonrası bacak ödeminin istatistiksel olarak anlamsız olduğunu tesbit ettik. Yaşlı hastalarda, doppler USG ile venöz fonksiyonların değerlendirilmesinden sonra safenektomi yapıldığında postoperatif bacak ödemi azalacaktır.
Abstract
In the elderly patients venous circulation functions impair due to insufficiency and degeneration of the venous valves. Saphenectomy in these eases may cause a prominent edema in the lower extremity. Evaluation of the lower extremity venous system is an efficient method. The condition of the deep veins, valve insufficiency and venous reflux can be evaluated by doppler ultrasound. This study was performed in patients who have undergone coronary artery bypass surgery. We searched whether there was edema formation following saphenectomy in patients who had preoperatively normal venous function. Doppler ultrasonic examination was performed to both lower extremity preoperatively and in the postoperative first month. Venous and arterial flow velocities were measured. Venous system was evaluated considering thrombosis, reflux and valve insufficiency. We found that the leg edema was statistically insignificant in patients who had normal preoperative venous function. The incidence of leg edema will decrease if saphenectomy is performed in the elderly patients after evaluation of venous function with doppler ultrasound.
