Özet

Amaç: Bu çalışmanın amacı, 1.daimi molar dişlere uygulanan paslanmaz çelik kronların dişeti sağlığı üzerine etkisinin klinik, mikrobiyolojik ve radyografik olarak değerlendirilmesidir. Gereç ve Yöntem: 9-10 yaşları arasında 20 çocuğun aşırı harabiyete uğramış ya da endodontik tedavi gerektiren simetrik alt 1. daimi molar dişleri çalışmaya dahil edildi. 20 adet paslanmaz çelik kron çinko fosfat siman Adhesor, 20 adet paslanmaz çelik kron fluoridji çinko fosfat siman Granitec Z2 ile yapıştırıldı. Sağlıklı antagonist 1 .daimi molar dişler kontrol grubu olarak seçildi. Tüm hastalara oral hijyen eğitimi verildi. Tedavi öncesinde ve tedaviden sonra 21. günde, 3., 8. ve 12 ayda klinik, mikrobiyolojik ve radyografik değerlendirmeler yapıldı. Dişeti cep sıvısında nötral proteaz enzim ölçümü için Periocheck (Pro-Dentec) ofis seti kullanıldı. Bu test kiti periodontal dokuların sağlığının tespitinde kullanılan klasik metotlarla karşılaştırıldı. Elde edilen veriler istatistiksel olarak değerlendirildi. Bulgular: Sonuçlar, paslanmaz çelik krona komşu dişeti dokusunun sağlığı ile sağlıklı daimi dişin dişeti dokusu arasında bir yıl sonra herhangi bir farklılık göstermedi. Periocheck (Pro-Dentec) ofis testinin sensitivitesi %87, spesifitesi %85 olarak bulgulandı. Sonuç: İyi uyumlanmış paslanmaz çelik kronların genç daimi dişlerde dişetinin sağlığı açısından güvenle kullanılabilir geçici restorasyonlar olduğu gözlendi.

Abstract

Objective: The aim of this study is to evaluate the effect of stainless steel crowns, placed on first permanent molars on gingival health with clinical, microbiologic and radiographic methods. Material and method: Symetric first permanent molar teeth of 20 children between 9-10 ages, with extensive caries or require endodontic treatment were used. Twenty stainless steel crown were cemented with Adhesor (zincphosphate cement) and symetric 20 stainless steel were also cemented with Granitec Z2 (zincphosphate cement with fluoride). Healthy, contralateral first permanent molar was selected as control tooth. All of the children received oral hygiene instructions. Periocheck chair-side kit was used to assay the presence of neutral proteases in clevicular fluid. This test kit was compared with traditional methods of detecting periodontal health. Clinical, microbiological and radiographic examinations of the teeth and surrounding tissue were performed before the treatment and in the 21 st day, 3, 8, and 12 months following the treatment. Periocheck chair-side kit was used to assay the presence of neutral proteases in clevicular fluid. Clinical, microbiological and radiographical measurements were evaluated statistically. Results: The results didn't show any difference between the health of gingival tissue adjacent to stainless steel crown restorations and healthy control tooth after one year. Periocheck had a sensitivity of 87% and a spesifisity of 85%. Conclusion: It was thought that the perfectly fitting stainless steel crowns on as interim restorations for the health of marginal gingiva.
