99 vakalık seride oral liken planusun kliniko-patolojik korelasyonun değerlendirilmesi

Özet
Amaç: Geriye dönük olarak planlanan bu çalışmanın amacı, oral liken planus (OLP) vakalarının klinik ve histolojik bulgularını gözden geçirmek ve OLP vakalarının klinik bulguları ile histopatolojik bulguları arasındaki korelasyonu araştırmaktır. Gereç ve Yöntem: 2000-2006 yılları arasında klinik ve histolojik olarak liken planus tanısı almış, toplam 99 vaka çalışmaya dâhil edilmiştir. Vakalar, yaş, cinsiyet, lokalizasyon ve histolojik görünüm açısından değerlendirilmiştir. Bulgular: Çalışmaya dahil edilen lezyonların % 75'i yanak mukozası, % 8'i dişeti, % 7'si dil, % 4'ü dudak mukozası, % 3'ü damak mukozası ve % 2'si retromolar bölge yerleşimlidir. Yaş ortalaması 47, erkeklerin kadınlara oranı 1:1,2 olarak saptanmıştır. Lezyonlardan 10 tanesi (% 10) eroziv tip OLP, 3 tanesi (% 3) büllöz tip OLP tanısı almıştır. Sonuç: Oral liken planus, oral mukoza epitelini etkileyen, nispeten sık görülen kronik inflamatuar bir hastalıktır. Klinik ve histopatolojik tanılar arasında % 65 uyum olduğu saptanmıştır.
Abstract
Aim: The goal of this retrospective study was to evaluate the clinical and histopathological features of oral lichen planus (OLP) and investigate the correlation between these clinical findings and histopathological features of the OLP cases. Material and method: Patients with the clinical and histological diagnosis of lichen planus, seen from January 2000 to December 2006, were included to this study. The following data were reviewed: age, sex, topography and the histopathological pattern of the cases. In a period of 6 years, 99 occurrences of OLP, confirmed by histopathologic findings in all, were observed. Results: Tha anatomic distribution of the cases involved in the study were; 75 % buccal mucosa, 8 % gingiva, 7 % tongue, 4 % labial mucosa, 3 % palatinal mucosa and 2 % retromolar zone respectively. The mean age of the patients is 47 and the male to female ratio is 1:1.2. Of all OLP cases in the study 10 cases were erosive type (10 %), and 3 were bullous type OLP. Conclusion: Oral lichen planus is a relatively common chronic inflammatory disorder affecting stratified squamous epithelia of oral mucosa, most commonly buccal mucosa, tongue and gingiva. There was a correlation of 65% between clinical and histopathological diagnosis. Oral lesions in OLP are chronic, rarely undergo spontaneous remission, and are potentially premalignant.
