Memenin Nadir Görülen Benign Proliferatif Lezyonu: Psödoanjiyomatöz Stromal Hiperplazi (PASH)

Özet
Amaç: Psödoanjiyomatöz stromal hiperplazi (PASH) meme stromasınınanastomozlaşan damar benzeri ince yarık şeklinde boşluklar ile karakterizebenign proliferatif bir lezyonudur. Etyolojisi ve patogenezi hala belirsizolmasına karşın, PASH’in genellikle miyofibroblastik kökenli bir neoplastiksüreci temsil ettiği düşünülmektedir. Bu çalışmanın amacı PASH tanısı alanhastaların klinikopatolojik özelliklerini ve cerrahi endikasyonlarınıdeğerlendirmektir.Gereç ve yöntemler: Gazi Üniversitesi Tıp Fakültesi Hastanesine başvuran veyapılan tetkiklerinde PASH tanısı alan 5 hasta retrospektif olarak incelendi.Hastalara ait yaş, şikâyet, aile öyküsü, radyolojik bulgular, tanı yöntemi ilgiliveriler geriye dönük olarak değerlendirildi.Bulgular: Hastaların 4’ünde memede büyüme gösteren kitle, 1’inde ise kanlımeme başı akıntısı şikayeti mevcuttu. Yaş aralığı 25-50, yaş ortalaması ise 35idi. Patolojik tanı, 1 olguda kor biyopsi, 2 olguda ultrason eşliğinde korbiyopsi, 1 olguda stereotaksik tel işaretli eksizyonel biyopsi ve 1 olguda isegeniş eksizyonel biyopsi ile konulmuştur.Sonuç: Radyolojik veya klinik olarak memesinde kitle saptanan hastalarınayırıcı tanısında benign olmasına ve nadir görülmesine karşın, PASH mutlakagöz önünde bulundurulmalıdır. Histopatalojik tanı, immünohistokimyasalboyamalarla desteklenmelidir.
Abstract
Objective: Pseudoangiomatous stromal hyperplasia (PASH), a benign proliferative lesion of breast stroma, is a mass lesion including anastomosing clefts reminiscent of blood vessels. It is still having an indefinite etiology and pathogenesis, and thought to represent a neoplastic process of myofibroblastical cell origin. The objective of this study is to evaluate the clinical and histopathological features and surgical indications of the patients diagnosed as PASH. Materials and methods: Five patients, attending to Gazi University Medical Faculty Hospital and diagnosed pathologically as PASH were examined retrospectively. The patients were re-evaluated by data concerning their age, complaints, family history, radiological findings and diagnostic methods. Results: Four patients have been presented with complaints of enlarging breast mass while the other with haemorrhagic nipple discharge. Ages ranged from 25 to 50 with the meanage of 35. Pathologic diagnosis was made by the examination of core biopsy in 1 case, ultrasound-guided core biopsy in 2 cases, stereotactic-guided excisional biopsy in 1 case, and wide excisional biopsy in 1 case. Conclusion: PASH, even though being benign and encountered rarely, must absolutely be taken into account in the differential diagnosis of patients with a breast mass detected radiologically or clinically. Histopathological diagnosis must be supported by immunohistochemical staining.
