Özet
Gebelik sırasında geçirilen akut toksoplazmozis enfeksiyonu, gelişmekte olan fetüs içinzararlıdır. Etiyoloji çok faktörlü olmasına rağmen, maternal enfeksiyon birincil olarak kontamine et veya su tüketimine atfedilir. Fetüse enfeksiyonun bulaşması yıkıcı nörolojik veoküler bozukluklara neden olabilir. Bu makalede, 37 yaşındaki bir annenin 4. gebeliğinden 3. canlı doğum olan konjenital toksoplazmozis olgusu sunulmaktadır. Anne gebeliği sırasında Toxoplazma IgM (+)’liği nedeniyle Spiramisin kullanmıştır. Yenidoğan bebeğin ilk değerlendirmesinde toxoplazma lehine bulguya rastlanmamıştır. BOS (beyin omurilik sıvısı) örneklemi alınan ve takip edilen olgunun PCR incelemesi pozitif gelmesi üzerine yeniden değerlendirilmiş, koryoretinit ve intrakranial kalsifikasyon bulguları ile tanısı onaylatılmıştır. Sonuç olarak, konjenital Toxoplazmozis sinsi ve yavaş seyirli bir hastalıktır. Erken yenidoğan döneminde net fizik muayene bulgusu olmayabilir. Bu hastalar detaylı incelenmeli, vücut sıvılarından örneklem gönderilmeli ve kesinlikle aralıklı takibi yapılmalıdır.
Abstract
Acute toxoplasmosis infections that develop during pregnancy can be detrimental to thedeveloping fetus. Although the etiology may derive from various factors, the primary causeof these maternal infections is the consumption of contaminated meat or water. Thetransmission of the infection to the fetus may result in devastating neurological and oculardisorders. In this article, we present a case of congenital toxoplasmosis that occurred on the 3rd live birth of a 37 year old mother’s 4th pregnancy. During the pregnancy, themother received Spiramycin as she was (+) for Toxoplasma IgM. In the initial evaluation of the neonate, there were no findings associated with toxoplasmosis. A CSF (cerebrospinal fluid) sample was taken and the patient was followed-up, a re-evaluation was conductedas the patient’s PCR analysis was positive; the diagnosis was confirmed by the presence of chorioretinitis and intracranial calcification. In conclusion, congenital toxoplasmosis is an insidious disease with a slow progression. Physical examination findings may not be apparent during the early neonatal period. These patients should be carefully examined, periodically followed up, and their bodily fluids should be tested
