Pediatrik Hastalarda Akut Gastroenterit Etkenlerinin Laboratuvar Parametrelerine Etkisinin Değerlendirilmesi

Özet

Giriş: Akut gastroenterit 0-18 yaş arası bireylerde sık görülen, mortalite ve morbiditeye neden olan önemli pediatrik infeksiyonlardandır. Akut gastroenterit etkenleri ve önemli laboratuvar bulgularının birlikte incelendiği çalışmaların sayısı sınırlıdır. Bu çalışmada; akut gastroenterit etkenlerini demografik bilgilere göre tanımlamayı ve laboratuvar değerleri üzerindeki etkilerini incelemeyi amaçladık. Materyal ve Metod: Çalışmamızda hastanemize Ocak 2018-Aralık 2020 tarihleri arasında başvuran 0-18 yaş arasındaki akut gastroenterit olgularında, etken, yaş, cinsiyet, mevsimsel dağılımı, hastanede yatış süresi ve bazı laboratuvar parametrelerindeki değişiklikler retrospektif olarak incelenmiştir. Hastalardan alınan dışkı örnekleri parazitolojik açıdan incelenmek amacıyla hem nativ-lugol, hem de serum fizyolojik ile incelenmiştir. Bakteriyolojik açıdan incelenmek için ise dışkı örnekleri Hektoen enterik ve Eosin Metilen Blue besiyerlerine ekilmiştir. Rotavirüs, adenovirüs ve Entamoeba histolytica tanısı için hızlı tanı kitleri kullanılmıştır. Bulgular: İncelenen 1685 akut gastroenterit etkeninin, 1554’ü (%92.22) viral, 127’si paraziter (%7.53) ve 4’ü (%0.23) bakteriyel olmak üzere üç ayrı etken kategorisinde incelenmiştir. Hastaların %46,2’si (779) kadın, %53,8’i (906) erkek hastalar olup yıl bazında yaş ortalaması 5,56 ± 3,96 olarak tespit edilmiştir. Viral etkenlerin %27,2’sinde adenovirüs, %72,7’sinde rotavirüs pozitif, bunların ise %10,36’sında aynı anda adenovirüs ve rotavirüs pozitifliği tespit edilmiştir. Paraziter etkenlerin, %74.01’inde E. histolytica, %7.87 hem E. histolytica hem de Giardia intestinalis aynı anda pozitifliği, %6.29 G.intestinalis ,%4.72 Blastocystis hominis,%2.36 Enterobius vermicularis, %1.57 oranlarında Taenia spp. ve Hymenolepis nana, %0.78 oranlarında ise Ascaris kisti ve Dientamoeba fragilis görülmüştür. Rotavirüs görülen hastalarda, lökositoz (%57.4), hiponatremi (%20.38) ve hipokalemi (%5.53), C-reaktif protein pozitifliği (CRP) (%83.52) diğer etkenlerde görülme sıklığından daha fazla tespit edilmiştir. Asidoz diğer laboratuvar bulgularından farklı olarak adenovirüs etkenli hastalarda (%62.5) daha fazla tespit edilmiştir. Sonuç: Akut gastroenterit tanılı çocukların büyük bölümünde etiyolojinin viral etken kaynaklı olduğu tespit edildi. Hastalığın minimize edilmesi için rotavirüs aşısı aşı programına dahil edilmelidir. Gereksiz antibiyotik kullanımıyla gelişen antibiyotik direncini önlemek için AGE düşünülen tüm hastalardan olabildiğince etken identifikasyonuna yönelik tüm tetkiklerin istenmesi ve laboratuvar tetkikleri yapılmadan tedaviye başlanmaması gerekmektedir.

Abstract

Introduction: Acute gastroenteritis is one of the important pediatric infections that dec common in people aged 0-18 years, causing mortality and morbidity. The number of studies examining the agents of acute gastroenteritis and important laboratory findings is limited. In this study, we aimed to identify the agents of acute gastroenteritis according to demographic information and to examine their effects on laboratory values. Materials and Methods: In our study, changes in the causative agent, age, gender, seasonal distribution, length of hospital stay and some laboratory parameters in acute gastroenteritis cases aged 0-18 years, who applied to our hospital between January 2018 and December 2020, were retrospectively examined. Stool samples taken from patients were examined with both nativ-lugol and saline for parasitological examination. In order to be examined from a bacteriological point of view, stool samples were planted in Hektoen enteric and Eosin Methylene Blue media. Rapid diagnostic kits have been used for the diagnosis of rotavirus, adenovirus and Entamoeba histolytica. Results: Among 1685 acute gastroenteritis agents, 1554 (92.22%) were examined in three different factor categories: viral, 127 parasitic (7.53%) and 4 (0.23%) bacterial. 46.2% (779) of the patients were female and 53,8% (906) were male patients, and the mean age on a yearly basis was 5.56 ± 3.96. Of the viral factors, adenovirus was detected in 27,2% and rota-positive in 72,7%, adenovirus and rotavirus were detected simultaneously in 7.6%. In the parasite, 74.01% E. histolytica, 7.87% both E. histolytica and Giardia intestinalis positivity simultaneously, 6.29% G.intestinalis , 4.72% Blastocystis hominis, 2.36% Enterobius vermicularis, in 1.57% study Taenia spp and Hymenolepis nana, in 0.78% Ascaris cyst and Dientamoeba fragilis were shown. In patients with rotavirus, leukocytosis (57.4%), hyponatremia (20.38%) and hypokalemia (5.53%), C-reactive protein (CRP) positivity (83.52%) were detected more often than in other factors. Acidosis, unlike other laboratory findings, was detected more often in patients with adenovirus (62.5%). Conclusion: In most of the children diagnosed with AGE, the etiology was determined to be caused by a viral factor. In order to minimize the disease, the rotavirus vaccine should be included in the vaccination schedule. In order to prevent antibiotic resistance that develops with unnecessary antibiotic use, all patients considered to be AGE should be asked for all possible active tests and treatment should not be started without laboratory tests.
