Diyaliz hastalarında egzersiz eğitiminin anksiyete ve depresyon ölçeği puanları üzerine etkisi

Özet

Amaç: Kronik böbrek yetmezliği sonucu diyalize giren hastalarda germe, gevşeme ve kalistenik egzersizlerden oluşan egzersiz programının 'Hastane Anksiete ve Depresyon Ölçeği' puanları üzerine etkilerini belirlemek amacıyla, bir ön çalışma planlandı. Gereç ve yöntem: Araştırmaya, kronik böbrek yetmezliği tanısı konan 8'i hemodiyaliz, 9'u periton diyaliz tedavisi gören 17 olgu alındı. Olgular araştırma ve kontrol grubu olmak üzere iki gruba ayrıldı. Araştırma grubundaki 9 hastaya fizyoterapist gözetiminde 8 hafta süreyle haftada 3 gün egzersiz yaptırılırken, kontrol grubundaki 8 hastaya ise bu egzersizler ev programı şeklinde verildi. Hastalara egzersiz programına başlamadan önce ve programı tamamladıktan sonra Hastane Anksiete ve Depresyon Ölçeği (HAD, Hospital Anxiety and Depression Scale) uygulandı. Sonuçlar: Yapılan bu ön çalışma sonucunda, gruplar içinde egzersiz sonrası anksiyete ve depresyon ölçeğindeki puanlar incelendiğinde, egzersiz sonrası araştırma grubunda anksiyete puanları açısından üç kişi normale dönerken, kontrol grubunda ise değişiklik elde edilmedi. Depresyon yönünden incelendiğinde ise, eğitim sonrası araştırma ve kontrol gruplarında birer kişi puanlama açısından normale döndü. Eğitim sonrası kontrol grubundaki olgulardan iki kişinin depresyon puanları sınırda normal olarak bulundu. Tartışma: Sonuç olarak, diyaliz hastalarında uygulanan egzersiz eğitimi, eğitim sonrası gruplar kendi içlerinde karşılaştırıldığında, istatistiksel olarak ortaya konamasa da, anksiyete ve depresyon puanlarını olumlu yönden etkilediği ve egzersiz eğitiminin diyaliz hastalarında psikolojik yönden iyileşmeler sağlayabileceği sonucuna varıldı. Konuyla ilgili daha fazla olgu üzerinde gerçekleştirilecek çalışmalara gereksinim vardır.

Abstract

Preliminary effect of exercise training on anxiety and depression scores in dialysis patients Purpose: This study has been planned to determine the effects of an exercise program containing stretching, relaxing and callisthenic exercises on patients receiving dialysis because of chronic fenal failure. Material and methods: Seventeen patients diagnosed as having chronic renal failure were included in this study. Eight of the 17 patients were treated with hemodialysis and nine patients with periton dialysis. The patients werfeSlvided into two groups as test and control. Nine patients in the test group exercised for eight weeks three times a week under the supervision of a physical therapist. The same exercises were given to eight patients in the control group as a home program. The Hospital Anxiety and Depression Scale was applied to the patients before and after the exercise program. Results: The results of this preliminary study showed thafollowing exercise while in the study group three patients anxiety score became normal, no such positive result was obtained in the control group. From the depression aspect one subject in the test group and one subject in the control group have changed to normal following the training. The depression scores of two subjects in the control group have been found near normal limits follpwing the training. Conclusion: When the study and control groups were compared the difference between groups was not statistically meaningful but positive developments have been obtained in the scores of anxiety and depression and it was concluded that exercise training psychologically supports the patients. The studies in which these results are evaluated on more patients are required.
