Özet

Amaç: Bu çalışmanın amacı musküler distrofili hastalarda evde uygulanan inspiratuar kas eğitiminin (İKE) süresinin inspiratuar kas gücü üzerine etkisini araştırmaktı.Gereç ve yöntem: Musküler distrofili 10 hasta çalışmaya katıldı. Solunum fonksiyon testleri, maksimal inspiratuar basınç (Pimax) ve maximal ekspiratuar basınç (Pemax) ölçümleri eğitimin başında, 6. ve 12. haftasında yapıldı. İKE’nde, solunum kaslarına sabit bir direnç veren eşik inspiratuar kas eğitim aleti kullanıldı. Eğitim yoğunluğu eğitim aleti ile başlangıç Pimax değerinin % 30’u olacak şekilde, haftalık eğitim yükü artışlarıyla belirlendi. On beş dakikalık İKE haftada 5 gün, günde 2 defa 12 hafta süre ile uygulandı. İki eğitim süresi arasındaki değişiklikler Wilcoxon testi ile karşılaştırıldı. Sonuçlar: Solunum fonksiyon testi ve Pemax sonuçlarında anlamlı farklılık yoktu. İnspiratuar kas gücü 6 hafta sonra anlamlı olarak arttı. On iki hafta sonunda ek iyileşmeler görüldü (p=0.02). Abstract

Purpose: The aim of this study was to investigate the effect of duration of home inspiratory muscle training (IMT) on inspiratory muscle strength in patients with muscular dystrophy.Material and methods: Ten patients with muscular dystrophy participated in our study. Pulmonary function tests (PFT), maximal inspiratory pressures (Pimax) and maximal expiratory pressures (Pemax) were measured before the therapy and after 6 and 12 weeks. A “threshold” IMT device which gives constant resistance on respiratory muscles was used for the training of inspiratory muscles. Training intensity was determined with the training device at loads of >30% of baseline Pimax and weekly training load increments. IMT sessions of 15 minutes duration twice daily, 5 days a week for a 12-week period were performed. The changes between the two training periods were compared by the Wilcoxon test. Results: No significant differences were noted in the results of PFT and Pemax values. Inspiratory muscle strength improved significantly after 6 weeks (p=0.01).
