Özet

Amaç: Bu çalışmanın amacı, konjenital ve edinsel çocuk amputelerde mobilite düzeylerini ve enerji tüketim seviyelerini belirlemekti. Gereç ve yöntem: Çalışmaya 8-17 yaşları arasında, 41 çocuk olgu katıldı. Konjenital ve edinsel olmak üzere iki gruba ayrılan olgular, protez yapım, eğitimi ve rehabilitasyon programına alındılar. Olguların ilk olarak ünitemize protez yaptırmak üzere başvurduklarında, ikinci olarak 3 haftalık protez eğitiminden sonra ve taburcu olduktan 6 ay sonra üçüncü değerlendirmeleri yapıldı. Ampute Mobility Predictor Scoring Form (AMPQ) ile mobilite değerlendirmeleri protezli ve protezsiz yapıldı. Enerji tüketimleri 6 dakika yürüme testi ile değerlendirildi. Sonuçlar: Olguların fiziksel özellikleri karşılaştırıldığında gruplar arasında anlamlı bir fark olmadığı görüldü (p>0.05). AMPQ açısından ilk ve 3 haftalık dönem sonrası değerlendirme sonuçlarında konjenital grup lehine anlamlı fark bulundu (p0.05). Enerji tüketim seviyeleri fizyolojik harcama indeksi ile incelendiğinde, 3 haftalık eğitim sonrasında konjenital grubun enerji tüketim seviyesinin daha az olduğu belirlenirken (p0.05). Tartışma: Mobilite seviyesinin gelişiminde, erken dönemde amputasyon nedeni ve seviyesi etkili olmakla birlikte, geç dönem sonuçları incelendiğinde çocuğun aktif olarak protezleri ile fonksiyonel uyumunun ve protez kullanma süresinin etkili olduğu sonucuna varıldı. Ayrıca, çocuğa özel tasarlanmış protezler ile mobilite düzeyinin artmasının enerji tüketiminin ekonomik kullanımını sağladığı düşünüldü.

Abstract

Purpose: Aim of this study is to investigate determining the mobility grade and energy consumption in congenital and acquired child amputees. Material and methods: A total of 41 children whose ages varied between 8-17 years participated in the study. Prosthetic fitting, prosthetic training and rehabilitation were given to the children. The assessments were performed in their first attendance, after three weeks of prosthetic training and 6 months after their discharge. Amputee Mobility Predictor Scoring Form (AMPQ) was applied with and without prostheses. Energy consumption was evaluated by six minute walk test. Results: It was seen that there was no significant difference in the physical characteristics of the two groups (p>0.05). The evaluation of data between the groups showed significant difference in favor of the congenital group with respect to AMPQ in the first and in the second assessment done three weeks after (p0.05).
