Abstract

Hereditary spastic paraparesis (HSP) is a genetic disorder that causes a gait disorderwith spasticity and weakness of the lower extremities. HSP causes marked disability and substantialsocioeconomic losses. The treatment goal is symptomatic: to improve the spasticity and gait disorderand to increase the patient’s quality of life. For the symptomatic treatment of HSP, oralantispastic medications, botulinum toxin injections, physical therapy, and surgery can be useful, but there is no consensus on the ideal therapy. This report shares a case that was unresponsive tovarious antispastic treatments, but showed marked improvement in the gait disorder with a reducedfall risk after administering an intrathecal baclofen infusion with a pump.
