Karpal tünel sendromu klinik tanısında provokatif testlerin önemi

Özet
Karpal Tünel Sendromumm (KTS) klinik tanısında median sinirin kompresyon veya stimülasyomma dayanan çeşitli provokatif testler kullanılmakta, bu testlerden en popüler olanları Phalen ve Tinel testi olarak bilinmektedir. Son yularda ters Phalen, karpal kompresyon testi, el bileğinin dinamik fleksiyon-ekstansiyon egzersizleri gibi diğer provokatif testler de uygulama alanına girmiştir. Bu çalışmada klinik ve elektrofizyolojik olarak KTS tanısı almış 31 hasta (15'i bilateral, 46 el) ve 24 sağlıklı kontrol olmak üzere toplam 70 ele sırasıyla Tinel, Phalen, ters Phalen, karpal kompresyon testi ve el bileği dinamikfleksiyon-ekstansiyon egzersizleri uygulanarak bu testlerin spesiflte ve sensitiviteleri araştırılmıştır. Timi testi sensitivitesi en düşük/akat spesifitesi en yüksek test olarak bulunurken karpal kompresyon testi sensitivitesi en yüksek fakat spesifitesi en düşük test olarak belirlenmiştir. Provokatif testleri pozitif olan KTS'lu hastaların median sinir distal duysal lalam değerleri ortalamaları ayn ayn incelendiğinde testler arasında istatistiksel olarak anlamlı farklılık gözlenmemiştir (p>0.05). Bazı otörlerce değerleri şüpheli kabul edilmekle birlikte rutin poliklinik muayenelerinde spesifitesi yüksek olan Tinel testi sensitivitesi yüksek olan diğer provokatif testlerden herhangi biriyle birlikte kullanılırsa, klinik tanıya önemli ölçüde katkı sağlanabileceği ve bu şekilde de nörofizyoloji laboratuvarlanna daha seçilmiş vakaların gönderilerek zaman vë ekonomik kaybın azaltılabileceği sonucuna varılmıştır,
Abstract
Various provocative tests that are dependent on the compression and stimulation of the median nerve are used in the diagnosis of the carpal tunnel syndrome (CTS). The most popular ones are known as Phalen and Tinel tests. In recent years, other tests like reverse Phalen, carpal compression test, dynamic flexion extemion exercises of the ivrist have come into practice. In this study Tinel, Phalen, reverse Phalen, carpal compression test and dynamic flexion extension exercises of the wrist were applied to 31 patients (15 bilateral, 46 hands) diagnosed clinically and electrophysiologically as CTS and 21 healthy controls a total of 70 hands, for evaluating the specificity and sensitivity of these tests. Tinel test was found to be the test with the lowest specificity and the highest sensitivity while the carpal compression test was the one with the highest sensitivity and the lowest specificity. No statistically significant difference was found between the different tests on comparison of the mean median nerve sensory latencies determined for each of the tests in patients whose provocative test were found positive (p>0.05). It was concluded that although the value of the provocative tests has been found doubtful by some authors, the combined use of the highly specific Tinel test and one of the other tests with high sensitivity in routine polyclinic practice will contribute significantly to making the clinical diagnosis and thus by way of referring more strictly selected cases to the neurophysiologic laboratory economic and time losses could be reduced.
