İntraserebral hemorajilerde yatak fazında alt ekstremite kemik ve mineral yoğunluğunun değerlendirilmesi

Özet
Vücut yükünü taşıma ve kas kontraksiyonlan gibi mekanik stimuluslann kısa bir süre için bile ortadan kaldırılmasının osteoporoza neden ola-bileceği saptanmıştır. Çalışmamızın amacı intraserebral hemoraji (İSH) geçiren hastalarda hastalığın erken dönemindeplejik kalça kemik mine-ral yoğunluğu (KMY)'nda meydana gelen değişikliklerin değerlendirilmesi, kas tonusunu ve serebral tutulumun KMY üzerine etkisinin incelen-mesi idi. Ayrıca, hemiplejiyi bir immobilizasyon modeli olarak kullanarak, kompresi/ve tensil mekanik güçlerin bir süre için ortadan kalkmış ol-masının trabeküler ve kortikal KMY üzerine etkisini araştırmayı hedefledik. Hastalık süresi ortalama 15,6 ± 3,6 gün (min 10; max 27) olan intraserebral hemoraji geçiren 25 hastanın hemiplejik ve sağlam kalça KMY du-al enerji X-ray absorbsiyometri yöntemiyle ölçüldü. Femur boyunun kompresi/, trokanter bölgenin tensil, Ward's üçgeninin her iki tip mekanik güçlerin KMY üzerine etkisini temsil ettiği kabul edildi, student's te testi kullanılarak emiplejik kalçanın/emir boynu, trokanter ve Ward's üçgeni KMY değerleri sağlam tara/ile karşılaştırıldı. İmmobilizasyon parametrelerinin (alt ekstremite Brunnstrom evresi, motor ve fonksiyonel kdça sko-ru, hastalık süresi), Ashworth skalasına göre derecelendirilmiş kas konusunun ve İSH özelliklerinin (lokalizasyon, lateralizasyon, dominant ve-ya non-dominant tara/tutulumu, inkontinans varlığı) hemiplejik kalça KMY ile ilişkisi korelasyon ve variyans analizleri ile araştırıldı. Hastala-rın yaşı, cinsiyeti, menopozal durum, vücut kitle indeksi (VKİ), hastalık öncesi fizik aktivite, diyetle kalsiyum alımı, sigara, alkol ve ilaç kullanı-mı gibi KMY'nu etkileyen/aktörler de göz önüne alındı. Abstract
It has been established that even a short-term lack of mechanical stimuli, such as weight-bearing and muscle contractions, can cause osteoporo-sis. The purpose of our study tvas to evaluate changes in the bone mineral density (BMD) o/theplegic hip as compared to that o/the uneffected side during early phase of intracerebral hemorrhage (ICE) ana to determine the role of muscle tonus and cerebral involvement upon BMD. Besides, using hemiplegia as a model o/immobilization, the role o/the compressive and tensile mechanical forces on trabecular and cortical BMD ivas investigated. Plegic and unaffected hip BMD o/25patients with ICHwas measured by dual energy X-ray absorbsiometry (DEXA) on the 10-27'b(mean 15,6 ± 3,6) day post-stroke. The femoral neck was accepted as representative o/the action o/the compressive, the trochanter o/the tensile and the Ward's triangle o/the action of both types o/mechanical forces upon BMD. Femoral neck, trochanter and Ward's triangle BMD values o/theplegic hip ivere compared with the espective ones o/the unaffected side by the Student's t test. The relations between the hémiplégie side hip BMD on the one hand and immobilization parameters (lower extremity Brunnstrom stage, motor and/unctional hip score, disease duration), muscle tonus grad-ed according to the Ashworth scale and ICH characteristics (location, lateralization, involvement o/dominant or non-dominant side, presence of incontinence) were evaluated by correlation and variance analysis, additional factors that could influence.BMD such as age, sex, menopausal status, body mass index (BMI), physical activity be/ore disease onset, smoking, use o/alcohol and medications were also taken into consideration.
