Hemiplejide solunumun fonksiyonel değerlendirilmesi

Özet
Bu çalışmada hemiplejik hastalarda mekanik ve radyolojik ölçümler ve solunum fonksiyon testleri ile solunum fonksiyonlarını değerlendirmek amaçlanmıştır. Serebrovasküler olaya bağlı bemiplejisi olan 40 hasta çalışmaya alınmış, aynı yaş grubundan mekanik re radyolojik değerlendirmeler için 20, ayrıca solunum fonksiyon testleri için 20 sağlıklı kişi kontrol grubu olarak değerlendirilmiştir. Tüm hasla ve kontrollerin üst ve alt göğüs, karın ve her iki hemitoraks ekspaıısiyonlan; ayrıca inspirasyon ve ekspirasvon akciğer giriftlerinde interkostal mesafeler, bemitoraks çaplan ölçülmüş ve hemidiyafram elenasyonlan değerlendirilmiştir. Solunum fonksiyon testi sonuçları beklenen değerlerle ne kontrol grubu sonuçlarıyla kaışılaştırtlmıştır. Hemiplejik hastalanıl üst ve alt göğüs re karın ekspansiyonlan kontrollere göre, ayrıca plejik taraf bemitoraks ekspansiyonu sağlam tarafa göre anlamlı olarak düşük bu-lunmuştur. Radyolojik ölçümlerde sağlam tarafla karşılaştırıldığında inspirasyonla interkostal mesafede artma plejik tarafta düşük bulunmuş; hemitoraks çapı, ekspirasyon ve inspirasyonda interkostal mesafeler arasında anlamlı fark saptanmamıştır. Radyolojik olarak plejik taraf hemidiyaframında elevasyon 12, sağlam taraf bemidiyaframında elevasyon 1 hastada gözlenmiştir. Hastaların solumun fonksiyon testi sonuçlarına göre ventilasyon performasında azalma ve restriktiftip solunum fonksiyon bozukluğu saptan-mıştır. Hcmiplejiklerde solunum sisteminin de etkilendiği ve bu hastaların rehabilitasyon programlarının solunum rehabilitasyonunu içermesi gerektiği sonucuna varılmıştır
Abstract
this study was performed to assess the respiratory functions in bemiplegia by using mechanical and radiological measurements and respiratory function tests. Forty patients with hemiplegia due to cerebronascular accidents uvre studied; 20 for the mechanical and radiological measurements and also 20 for the respiratory function tests, age matched healthy subjects uvre studied as the control groups. All patients and controls upper and lower chest, abdominal and both bemitbomcal expansions were measured. Intercostal distances and bemithoracal diameters in both inspiratoiy and expiratory chest roentgenograms were measured and hemidiaphrag-matic derations were evaluated. Respiratory function test results were compared with the expected values and the control group re-sults. In hémiplégies, upper and lonvr chest and abdominal expansions uvre significantly lower than those of the control group and bemithoracal expansions were significantly lower in theplegic side compared to the uniiwolved side. In radiological measures, when compared to uniiwolved side increments of intercostal distances with inspiration were lower in theplegic side, no significant diffe-rences uvre obsenvd between hemitboracal diameters and inspiratory and expiratory intercostal distances. Uierc was elevation of hemidiaphragms on theplegic side in 12 patients and on the normal side in 1 patient. Respiratory function test results revealed decreased renlilatory performances and restrictive respiratory impairment. It has been concluded that respiratory functions as well are affected in hemiplegia and the rehabilitation.program of these patients must include pulmonary rehabilitation.
