Özet
Romatoid Artrit (M) etyolojisi bala tartışmalı olan, otoimmünitenin ağırlıklı olarak suçlandığı, kronik progressif inflamatuar bir hastalıktır. RA . seyri sırasında değişik derecelerde tiroid dis/oksiyonu görülebilmektedir. Bu araştırma RA'lı hastalar ile tiroid disfonksiyonu arasındaki ilişkiyi ortaya koymak için planlanmıştır. Daha önceden klinik ve laboratuvaryöntemler ile M(n = 50) vegonartroz (n - 30) teşhisi almış 80 basta çalışmaya kabul edildi. RA'h hastalar Grup I, gonatrozlu hastalar ise Grup II (kontrol grubu) olarak ayrıldılar. Hastaların hiçbirinde daha önceden ortaya konulmuş tiroid disfonksiyonu ya da tiroid hastalığı öyküsü yoktu. Her iki gruptaki hastalarda, total T3, total T4, serbest T4, T uptake, Tiroid Stimulan Hormon (TSE), Anti Nükleer Antikorlar (ANA), Anti Mikrozomal Antikorlar (AMA), Anti Tiroglobülin Antikorlar (AT) ile romatoid Faktör (RF) incelendi. Grup I'deki 24 hastada (%48) tiroid fonksiyon testlerinden en az bir tanesinde anormallik tesbit edildi. Bu hastalarda 2'si hipotiroidi, geri kalan 22 hasta ise herhangi bir klinik tabloya sokulamadığı için Hasta Ötiroid Sendromu (HÖS) olarak değerlendirildi. Kontrol grubunda ise tüm hastaların tiroid fonksiyon testleri normal olarak değerlendirildi. Tiroid disfonksiyonu ile basta yaşı, RA hastalık süresi, RF varlığı, ANA, AT ve AMA arasında ilişki saptanamadı.
Abstract
Rheumatoid arthritis (RA) is a chronic, progressive, inflammatory disease with an unknown ethiology in which autoimmunity is mostly suspected. During the progression of this disease, thyroid disfunction can be determined. This study is planned to find out the relation between RA and thyroid disfunction. Fifty patients with RA and thirty patients with gonarthrosis: totally 80 patients were included in the study. The patients with RA were in the Group I, while the others in Group II (as control group). None of the patients had a history of thyroid dysfunction in the anemnesis. In the blood samples of all the patients in the groups, total 73, total T4,free T4, T uptake, thyroid stimulating hormone (TSH), antinuclear an-tibody, antimicrosomal antibody, antithyroglobulin antibody and rheumatoid factor was evaluated. Twenty-four patients (48%) in the Group I had at least one abnormality in the thyroid Junction tests. Two of these patients had hypothyroidism and the rest (22 patients) were diagnosed as euthyroid sick syndrome as the abnormality in their tests did not fit any other clinic appearance. All the patients in the control group had nor-mal values in the tests. No relation was found between the age of the patients, the duration of the illness, antinuclear antibody, antimicrosomal antibody, antithyroglobulin antibody and the thyroid disfunction.
